Abstract : | [Introduction] The practice of dental implant therapy in Japan was rapidly introduced in the 1960's, primarily by clinicians. At that time the fibrous capsule which formed around dental implants was considered a substitute for the periodontal ligament (PDL), but there were objections to that interpretation. Later, rapid advances were made in dental implants as a result of better understanding of the response of bone tissue to various materials, improvements in surgical technique, etc., and stronger support modalities, such as osseointegration and biointegration, were achieved at the material-jaw bone interface. These support modalities, however, differed considerably from the support provided to natural teeth by the PDL and became a source of mechanical imbalance in the bite-chew system. While the results of biomechanical studies performed in the past have shown that such dental implants are excellent in terms of static load bearing ability, many problems have been pointed out, e.g., substantial transmission of impact forces which becomes a cause of bone destruction, poor mobility, accumulation of functional stress on the natural dentition, and, lacking sense organs, inability to control excessive force. |
