Abstract : | Space deficiency in a crowding malocclusion is a major indication for premolar extraction treatment, although the indication of extraction has become established among dental practitioners without scientific evidence with a biological basis. It has been suggested that terminal molar or second-molar extraction treatment is an alternative treatment method that is appropriate in certain cases. Class I malocclusions with crowding and anterior cross bite in growing patients can be treated in the first phase with a removable expansion appliance followed by second-phase treatment. We report here a case in which orthodontic treatment was discontinued for 9 years. After that interval of time, the posterior discrepancy was more severe than before and the crowding was aggravated. We therefore did a second-phase treatment consisting of extracting all four third molars and applying an edgewise technique with multiloop edgewise archwire(MEAW)to correct crowding malocclusion. We describe the process of managing this patient with tooth crowding; the treatment outcome was acceptable and relatively stable. We conclude that eliminating posterior discrepancy by extracting all posterior molars introduces a new modality for treating crowding malocclusions. |
