Abstract : [Introduction] White lesions of oral mucous membrane are relatively common. While most are innocent and require no treatment, an important minority (roughly 3%) are potentially dangerous if ignored and allowed to progress. Accordingly, it is important for the clinician to have at his or her disposal a reliable chairside means for distinguishing which white lesions are worrisome and which are not. Armed with such expertise, the clinician will be able to categorize these lesions on a clinical basis and in so doing have the means to assure patients that obviously reactive white processes (the vast majority) are innocent and need not be of further concern. In contrast, those that are suspicious can be pursued and definitively diagnosed through subsequent biopsy. Accurate identification of a premalignant process or frank malignancy in early stages yields an excellent prognosis for complete recovery without disfiguration, loss of function or significant operative discomfort. On the other hand, failure to recognize or diagnose a suspicious white lesion with malignant potential can allow progression to the point where treatment is either not possible or ineffective.
