Abstract : There is no formal terminology for the so-called "gakukansetsusho" that is regarded to be a disease incidental to civilization. Currently, a terminology "temporomandibular dysfunction" is commonly used. In recent years, the cases of functional abnormality in the stomatognathic system attributed to the dysfunction in mandibular joints and in the muscles in the lower portion of the head have been on the increase. Under these circumstances, these problems must be tackled urgently. In 1986, the Japanese Society for Temporomandibular Joint classified the symptoms of temporomandibular arthrosis into five types. Among them, the Type III mandibular intraarticular dysfunction is characterized by a structural malfunction of the joints. This malfunction is caused mainly by a morphological change in the articular disks, dislocation of the disks, and an abnormal shift of their positions vis-a-vis the heads of mandible. These changes produce a clicking joint noise as a result of the movement of articular disks when the mouth is opened. At times, these changes cause difficulty in opening the mouth, a symptom known as locking. The number of clinical cases of intraarticular dysfunction is increasing. In recent years, remarkable progress has been made in the basic research on mandibular dysfunction, particularly on the mandibular intraarticular dysfunction, and in clinical practices along with a similar improvement in diagnosis equipment and the establishment of treatment method. Under these circumstances, I consider it will be significant to those involved in dental profession to introduce how basic and clinical researches, diagnosis and treatment are being conducted and what results have been produced from these researches. And I hope that these feature articles on the mandibular intraarticular dysfunction in this issue of The Bulletin of Kanagawa Dental College will contribute to the promotion of the method of diagnosis and treatment in basic as well as clinical practices in dealing with the complexity of the dysfunction.
