Abstract : [Abstract] In the present study, the effects of local application of tetracycline-immobilized cross-linked collagen film (TC film) and root debridement used alone or in combination were investigated by assessing changes in a pocket depth (PD), which is considered to be an important factor influencing the prognosis of periodontitis. Thirty-three sites with a periodontal PD of 4-6 mm and 28 with a depth of 6 mm or more, in 15 patients with periodontal disease showing good supragingival plaque control, were randomly allocated into the following 4 treatment groups: TC film applied at one-week intervals for 4 weeks; one scaling treatment; both the above treatments in combination; and, untreated (control). Therapeutic effects were assessed both clinically and microbiologically. A marked decrease in PD and the number of microorganisms in the periodontal pocket were noted for sites with less than 6 mm PD treated with either scaling or a combination of scaling and TC film application. No differences among the treatment groups were found for any other parameters evaluated. The sites with a pocket depth 6 mm or more treated with the combination of scaling and TC film exhibited an increase in attachment gain, a decrease in bleeding on pocket probing, and a decline in the number of spirochetes. These results demonstrate that local application of TC film and root debridement are both useful in the treatment of periodontal disease, and that their combined use is particularly useful in the treatment of sites with deep pockets.
