Abstract : [Abstract] Accurate determination of the severity and chronic nature of a patient's liver disease and the correction of reversible risk factors both help in reducing operative mortality and in avoiding perioperative complications in patients with liver disease undergoing surgery. As ongoing acute hepatocellular necrosis and Child's C cirrhosis are frequently associated with perioperative complications, patients so diagnosed should avoid elective surgery. Chronic hepatitis C should be treated with interferon before surgery to reduce or eliminate the symptoms of hepatitis and viremia. Emergency surgery for patients with liver disease is challenging, since a preoperative evaluation of the status of the liver may be limited and perioperative mortality is high. Aggressive support in an intensive care unit is essential in this group of patients. The risk of exposure to surgical personnel from patients' blood during surgery can be reduced by the practice of double gloving, the use of water-proof garments, and the use of face shields.
