Abstract : [Abstract] Hyperdivergent (high mandibular plane angle) type Class II malocclusion is one of the most difficult cases to treat orthodontically. The reason for this is that the skeletal problems in this malocclusion involve small, retruded mandibles accompanied by backward rotation. In addition to this, cases have a high frequency of temporomandibular joint (TMJ) dysfunction. In this report, we demonstrate a treatment sequence that will succeed for an adult hyperdivergent type Class II malocclusion accompanied by dysfunction of the craniomandibular system. This patient required control of mandibular position and improvement of the condylar movement which was limited by a locked joint and an anteriorly displaced articular disk. Treatment results indicated that the malocclusion was corrected by the control of the occlusal planes and a relatively stable occlusion was established. Orthodontic treatment succeeded in obtaining a symptom free functional craniomandibular system in spite of persistent displaced articular disks at treatment completion.
