Abstract : [Abstract] We investigated the sphenomandibularis (SM) muscle, an important member of the family of masticatory muscles first reported by Dunn et al. (1996). We used four cadaveric specimens, sagittal slices of the magnetic resonance imaging (MRI) from 40 clinical patients, and the muscle activity recordings from adult volunteers. This muscle originates from the anterior surface of the facies temporalis of the sphenoid bone (posterior part of the orbit) and enters the internal oblique line of the mandible. Electromyography shows that this muscle plays an important role as a member of the set of masticatory muscles, especially in limiting the eccentric movement of the mandible. Additionally, our clinical examinations suggest that the retro-orbital pain experienced by some TMD patients is closely related to the hypertonicity of the SM muscle. We hypothesize that stiffness of the sphenomandibularis muscle affects mandibular movement and produces TMD signs such as retro-orbital pain.
