Abstract : [Points] 1. "A Physician's Charter" defines medical professionalism as the basis of medicine's contract with society. This contract demands constantly updated knowledge and skills. 2. Currently there is a gap between existing evidence and actual medical practice; there is an even larger gap in current dental practice. 3. Randomized controlled trials have the highest clinical relevance, and meta-analysis provides synthesized effect measures with the highest validity. 4. Because heuristics sometimes leads to severe and / or systematic error, we should rely on quantitative effect measures and a probabilistic approach to clinical decision-making. [Evidence-based medicine (EBM) and evidence-based dentistry (EBD)] The practice of EBM is the process of life-long, self-directed learning, in which caring for our own patients creates the need for clinically important information about diagnosis, prognosis, therapy, and other clinical and health issues. The practice of EBM consists of five steps: 1) converting information needs into answerable questions; 2) tracking down the best evidence with which to answer these questions; 3) critically appraising the evidence for its validity and importance; 4) integrating this appraisal with clinical expertise and patient values to apply the results in clinical practice; 5) evaluating performance. The same five steps apply to EBD . In step 2, electronic databases are used to locate available evidence and in step 4, the appraisal is integrated with the patient's perceived needs and with restrictions imposed by the health care system or health insurance system.
