Abstract : [Abstract] A Skeletal Class III reversed occlusion morphologically defined by mandibular prognathism, maxillary retrognatism or a combination of the two is often accompanied by mandibular lateral deviation (MLD) and in most cases is considered an indication for orthognatic surgery. Posterior discrepancy, i.e. tooth-to-denture base discrepancy in the posterior area resulting in crowding distal to the first molars, has not been regarded as an etiological factor causing lateral mandibular displacement. In this respect, it is important to recognize the effects of the flattening of the occlusal plane (OP) as well as variations in the vertical dimension on both sides due to the unilateral over-eruption of the molars caused by the squeezing-out effect of posterior discrepancy. This affects the vertical relationship of the dentition providing interferences in occlusal function and contributes to the malposition of the mandible, associated with a decreased vertical dimension and higher positioning of the mandibular condyle on the displaced side. Therefore, it should be emphasized that elimination of the posterior discrepancy prior to the differential vertical dimension control of the occlusal planes is of paramount importance in the planning of any treatment of Class III malocclusions with MLD. Two clinical case examples will illustrate how premature occlusal contacts have resulted in lateral mandibular deviation in dentitions showing posterior discrepancy. Orthodontic treatment was achieved with the reconstruction of the deviant occlusal planes using multiloop edgewise arch wires subsequent to the extraction of the third molars to eliminate the posterior discrepancy in order to reestablish the appropriate mandibular position and to improve the lateral mandibular displacement and associated symptoms relating to temporomandibular joint dysfunction with no surgical intervention.
