Abstract : | [Abstract] This paper presents a new approach to skeletal fixation using an external plate following tooth-borne rapid maxillary expansion (RME). The external plate system, named Anchor Lock, was originally developed for orthodontic absolute anchor by using a craniomaxillofacial implant. It consists of an anchorage and supportive part composed of a double-Y shaped plate and four titanium screws, respectively.These 2.0-mm diameter screws were inserted trans-mucosally and their heads rigidly locked in the holes of the external plate. Because the plate is supra-mucosally suspended from the palate, it is easily fixed and removed under local anesthesia with minimal surgical invasion. RME was performed on 19 patients with transverse maxillary deficiency using the conventional tooth-borne type appliance with a Hyrax screw. To retain the maxilla in the expanded state, immediately after the RME appliance was removed, an external plate was fixed to the palatal vault using titanium screws. In addition to ensuring bone fixation, the external plate is also used as an orthodontic anchor for further teeth movement after RME. In all patients, the Anchor Lock plate successfully secured and retained maxillary expansion by tooth-borne RME. Ou r observations suggest bone fixation using an external plate after tooth-borne RME as an alternative to conventional RME methods. |
