Abstract : | 諸外国に類を見ない速さで高齢社会を迎えている我が国において, 現在, 高齢者の健康の維持増進を目的として高齢者の健康管理に, より一層の努力が払われており, 顎口腔領域においても, 咬合や咀嚼機能の回復改善や歯周疾患などの口腔内の管理が増々必要とされる1,2). 中でも感染症に対する免疫抵抗力の衰えた高齢者に対して専門的に徹底した口腔ケアが望まれる. しかし, 床義歯を装着した患者の口腔ケアの実態をみると, 健常者であっても十分とはいえず, 要介護者に至っては極めて不良である3). 床義歯の装着がプラークの堆積を促すこと4)は周知の事実であり, 義歯に付着したプラークが原因で細菌感染を生じ, 誤嚥性肺炎やその他の全身的な疾病を引き起こす危険性が数多く報告されている5~10). そのため, 多くの高齢者が装用している義歯を清潔に保つことは大変重要であり, 義歯をプラークの付着しにくい状態に改質することは, 高齢者の口腔内環境を良好に保ち, さらに健康維持への一助となると考えられる. |

Abstract : | Coating of fluoride polymer a the denture by plasma polymerization may have a high preventive effect against staining and bacterial plaque accumulation. Especially, the vapor deposition polymerization (VDP) method in plasma polymerization, which can directly synthesize a thin film from two or more gaseous monomers under a dry process, may be able to produce an effective fluorine based antibacterial hydrophobic film. The present study evaluated the effectiveness of this film coated on a denture using the VDP method. A heat-curing acrylic resin was used as denture base material, and rectangular PMMA samples (30 × 10 × 2 mm) were prepared and smoothly finished. The surface of PMMA was modified by the following three ways:1) 2,2,3,3,3-pentafluoro-1-propanol monomer (the PFP group), 2) 1,3,5,7-tetramethylcyclotetrasiloxane monomer (the TMCS group), and 3) a monomer mixture of PFP and TMCS (the MIX group). Non-treatment specimens were used as controls. All modifications were done with an R.F. power of 120 Watts or 200 Watts. Surface characteristics, such as surface roughness, contact angles, knoop hardness, and △E^* ab, were evaluated. Using a Fourier Transform Infra Red spectroscope (FT-IR) and an Electron Plobe Micro Analyzer (EPMA) detected the presence of fluorine (F) and silicon (Si) . The data were also analyzed statistically using ANOVA and Fisher PLSD. The following results were obtained: 1. The surface roughness of all modified groups indicated significantly lower values as compared with controls (p<0.05). 2. The contact angles of the MIX group were significantly higher than those of controls (p<0.05), and they also indicated significant, lowest values of surface-free energy among the modified groups (p< 0.05). 3. Knoop hardness of all modified groups showed a significant decrease compared with the controls, with an increase of R.F. power (p<0.05). 4. Modification using the VDP method showed no influence on surface color. 5. EPMA analysis showed the presence of F and Si element on the modified surface of the MIX group, and FT-IR spectra analysis identified a C-F and Si-O bond. The present findings indicated that the modified surface of PMMA by the VDP method using mix monomer (PFP and TMCS) may possess plaque formation inhibiting ability and plaque detachment efficacy in the oral environment. |
