| Abstract ： | 「諸言」矯正歯科臨床において, 歯の先天欠如を伴う不正咬合にたびたび遭遇する. 特に下顎前歯の先天欠如は近年増加傾向にあると報告されている. 下顎前歯が1本先天欠如したthree incisors症例に対して, 矯正歯科治療を行う場合多様な治療計画が立案される. 単純に排列のみを行う方法, ストリッピングを併用して排列する方法, 補綴処置を行う方法, さらにAngle I級の咬合の確立や正中線を一致させることができるか否かによっては変則的な抜歯を選択することもあり, 治療方針を決定するにあたってはtooth size ratioを考慮することが重要となる. また, 骨格性下顎前突症例に対して矯正歯科治療を行う場合, 矯正単独治療が困難であれば外科的手術の併用も考慮する必要がある. 外科的矯正手術を選択した場合, 術前に上下顎前歯の歯軸角を修正する必要があり, 下顎骨の後退量が十分得られるような状態にしなければならない. |

| Abstract ： | [Abstract] Congenitally missing teeth is often encountered in everyday clinical practice. When restoring a satisfactory occlusal relationship using conventional orthodontic treatments is considered to be difficult due to marked imbalance between the upper and lower jaws, surgical orthodontic treatments are performed. This is a case report of a 27 years and 10 months old female patient who was concerned about antero-posterior crossbite at the time of first examination. We treated mandibular protrusion with congenital missing tooth in mandibular left lateral incisor by non-extraction and surgical orthodontic treatments. This treatment successfully improved the condition both morphologically and functionally. As a result, we were able to obtain a favorable occlusion and this was still maintained 2 years and one month after the treatment. |
