Abstract : 「I 緒言」 どんなに優れた充填物や補綴物が施されても, その基礎工事とも考えられる根管治療や, 根管充填が不確実であっては, いわゆる"砂上の楼閣"にひとしくなる. 根管充填は, 根管治療の最後のしめくくりであり, その良否は予後に大きく影響を来たす. すなわち, 日常の臨床において, 以前根管充填を施したことのある歯牙のX線観察を行なってみると, 非常に多く根尖病巣の存在が認められる. このことは, 根管治療ないしは根管充填の困難さを如実に示すものであるが, われわれは, この困難性に打ち克って, より適確な治療を行なうように努力をしなければならない. 根管充填を適確に行なうには, 根管の拡大および清掃を充分に行なうとともに, すぐれた根管治療剤や根管充填剤の選択が必要であるが, それらの究極として, 歯根を歯牙周囲組織に対して, 無害なものとすることが必要である.

Abstract : The successful root canal therapy depends on the thorough sterilization of root canal. The most of the empirical methods to detect the presence of microorganisms are without foundation. On the other hand, the smear and the culture methods are the scientific processes to examine the bacterial status of the root canal. However, the smear technique is not practical for the general practitioner because of its messy procedures such as microscopic identification of microorganisms and the inability to find out the bacterial vitality. In this point of view, the use of culture methods is more than repays the dentist for the small amount of time and the effort necessary to carry out the procedure. In using the culture technixue for endodonitic treatment, the medium and incubator should be practical. Our efforts had been made to improve the media and the incubator. We found that the combination uses of "ampoule culture medium" and the thermostatically controlled small electric incubator adequate for the average dental practice.
