| Abstract ： | 「I)はじめに」根管治療を成功させるためには, (1)徹底した根管の清掃と拡大, (2)根の消毒, (3)気密な根管充填の3原則が守られねばならない. これらのいづれでも欠けるようなことがあれば, 根管治療の予後を低下させることは疑いもない事実とされる. しかしながら, 根管処置の過程において, 学生や初心者, また稀に熟練者においても, 不幸にして根管用小器具の破折事故をおこすことがある. 根管内の破折片が除去されないで根管に残存していると, 根尖孔までの十分な根管拡大と清掃が行なえず, また完全な根管消毒はもちろん, 緻密な根管填塞も施行が困難となるため, 前述の根管治療の原則が遵守されず, 予後が危ぶまれる. したがって, 破折片をなんとかして除去せねばならないということで, 従来から, 機械的な除去方法, 化学的に腐蝕して除去する方法などが, いろいろと報告されている. しかしながら, 機械的に把持困難な破折片, とくに, 根管中央部より根尖近くに食い込んだように折れ込んでいるリーマー類の機械的除去は殆んど絶望的と考えられる. |

| Abstract ： | [Abstract]: Clinical investigation was conducted on the prognosis of endodontically treated teeth, in which intracanal instruments were broken. In these cases, the root canals were filled without removal of the fragments. There were 10565 cases treated endodontically by undergraduate students of the Kanagawa Dental College during the period from 1974 to 1977, and all of these cases were evaluated. Among these cases, broken instruments in the root canals were found in 104 cases. Radiographic and clinical examinations were performed in 44 cases available for recall. These cases were divided into two groups, 18 cases of pulp extirpation and 26 cases of infected root canal. In the former, 16 (76%) were determined to be successful, whereas in the latter, 22 (79%) were successful. The results found is this clinical investigtion shows a relatvely high rate of success in regards to treatment that is given when pieces of endodontic instruments are left in the root canals. |
