Abstract : 「抄録」 咀嚼や咬合によって歯に加わった力は顎骨に伝達され, その力が生理的限界を越えて加えられた場合に重要な緩圧作用を示すものは歯根膜であると言われている. これらについての研究は多方面にわたって行われているが, 補綴領域への応用は未だ解決されていない. また局部床義歯学の立場からは残存歯や歯槽堤に負担過重を生じさせないように維持装置, 義歯床, 咬合に関する多くの研究が先人によってなされている. これらの補綴学的研究も人工模型実験であったり, 歯を不動の要素として行っているものが主であるので, 著者は咀嚼力や咬合力がどのように伝達し頭蓋において分散しているのか, また従来から言われている歯周組織の緩圧能力がどの程度であるのかなどの基本問題を解明する必要性を感じて本研究に着手した. 歯に加わる力を衝撃として考えるとき, 時間・大きさ及び力の質を考慮することが必要である. そのため著者は加振力に動的な力として加速度を応用した. 一定な加速度を加振力としたとき, その伝達経路及び減衰は電気的に監視及び記録が容易に行いうる. 本実験では入手しやすい被験個体としてイヌの頭部を使用し, 直接ネジ止めによって加速度計を各測定点に固定し, 伝達状況を正確に把握できるようにした. 測定点はイヌ頭部8ヵ所で各々上下, 前後, 左右方向の3方向を測定した. 条件は加振点を上下顎右側犬歯2ヵ所とし, 加振側と反加振側について, 各々軟組織が付着したもの及び軟組織を可及的に除去したものについて測定した. また咬合や咀嚼時には上下の歯が接触して力が分散されるが, 咬合時の力の大きさや内容, さらに上下顎への配分状況が未知であるので, 歯の非接触状態で既知の同一で一定であるランダムノイズの一種類であるピンクノイズを強制振動として加えて伝達ならびに減衰状態を把握するように試みた. ランダムノイズによる加振は, 自然界に発生している振動に類似しており, また単なる一つの量として計測が可能なため, 本実験における減衰量の測定の様な, 相対比較をするには有効で客観的な方法であると考えた. 測定結果をまとめると次のようになる. 1. 歯軸方向の加振では相対的に左右方向の減量が最も多い傾向が示され, 次いで前後, 上下方向の順であった. 2. 振動伝達の基本は骨形態にしたがって行われ, いずれの骨も厚さより幅の広い方や長い方に向かって伝達される傾向が認められた. 3. 軟組織は振動減衰の基本パターンを変える程の影響はないが振動のダンピングの大きな要素の一つであり, 振動伝達における各方向間の有意性を変える因子であった. 4. 加振歯直下の減衰量が他の測定点と比較して少なく, 本実験における全帯域の加振においては加振歯直下(歯槽底)の減衰性は微少であった. 5. 加振側と反加振側との減衰量の差は軟組織を除去したもので, 加振側測定点での減衰量が大きく, 軟組織が付着したものでは, 軟組織の影響を受けて反加振側での減衰量が大きかった. 6. 歯に加わった衝撃は上下顎骨のみならず頭部全体で分散され, その分散の最も大きな比重をしめる部位は脳頭蓋部であった. そして上下顎いずれの歯に振動が加わっても一顎で分散されるのではなく, 頭部全体で分散された. 脳頭蓋部の平均減衰量は12.3~16.0dB(1/4.1~1/6.3)で, 実測値最高減衰量は頭頂骨の25dB(1/17.8), 推定値最高は21.5dB(1/11.9)であった.

Title : Transmission Route and Attenuation of Impact on Tooth

Abstract : Force by mastication of occlusion is transmitted from teeth to the jaw, and when the force exceeds a physiological limit, the periodontium is said to show a significant stress breaking action. The matters have been studied from various aspects, but the application to prosthodontics has not been established. In the reserch of partial dentur, there have been many studies on retainers, denture base, and occlusion so as not to load the remaining teeth and alveolar ridges excessively. Most of these prosthodontic studies are, however, of artificial model experiment type or those in which teeth are treated as an immovable matter. It was noted that such fundamental problems as the transmission of the force of mastication and occlusion and its distribution in the skull and the degree of the ordinarily-descrbed stress breaking action of the periodontal tissue must be resolved, and the tests reported here were performed. Considering the force to the tooth as an impact, the time, the strength and the nature of the force must be noted. Therefore it was applied that the concept of acceleration to the vibration-loading power. Providing that a constant acceleration is the vibration loading power, its transmission route and the attenuation can be electriclly monitored and recorded easily. In this examination a canine head readily available was used as the test sample and the transmission of the force was exactly observed by fixing an acceleration meter directly to each test point with screws. Eight test points were set on the canine head to measure the power in the three directions of up-down, anterior-posterior and right-left. The vibration-loading point was set at two points, each of the right canine teeth of the upper and the lower jaws; under these conditions the power was observed in the vibration-loaded side and non-loaded side with the soft tissue attached and without the soft tissue, which had been removed as quickly as possible. In occlusion and mastication, upper and lower teeth are attached to each other to disperse the power, but the strength and the nature of the power and its distribution in the upper and the lower jawa are unknown. Then pink noise which is a sort of random-noise and is a known same and constant noise under a non-attached condition, was added as a forced vidration to observe the states of transmission and attenuation. Vibration caused by random-noise resembles vibration occurring naturally and can be measured as a single quantity at a measuring point, and thus it will serve as an effective and obqective means of comparison. The results are as follows; 1) When vibration was applied along the axis, the attenuation of the left-right detection was the highest, followed by that of the anterior-posterior direction and that of the up-down direction in this order. 2) Vibration was basically transmitted depending on the shape of the bone; in any bone it was transmitted more along the width and the length than along the thickness. 3) The soft tissue did not change so largely the basic pattern of the attenuation of vibration, but was one of important factors for damping of vibration. 4) Vibration to the teeth dispersed not only to the upper and the lower jaws but to the whole head, and dispersed most to the cerebral skull. Whenever an impact was given to any tooth in either the upper or the lower jaw, it dispersed not only in the jaw but in the whole head. 5) The observed maximum attenuation was 25.0dB (1/17.8) at the partial bone and the theoretical maximum attenuation was 21.5dB(1/11.9); the average attenuation at the cerebral cranium which appears to be the center of dispersion was 12.3-16.0dB (1/4.1/6.3)
