Abstract : 「緒言」 歯科領域で遭遇する感染症, とくに重症感染症(腫瘍性潰瘍)は時としてshockに移行する. その要因は抗生物質の広汎な利用と乱用による耐性菌の増加にもとづいており, septic syndromeの致死的原因がendotoxinによるため, 感染症の治療にseptic shockの概念が導入されるようになった. これは抗生物質の乱用初期に全く一致しており, 以来, septicなshock syndromeに対する研究はさらに活発になされるようになった. ことに近年, shockに関してはその増悪・非可逆性が問題視され二つのhumoral factorの存在が衆目の認あるところとなった. すなわち, shock時に産生され, 特異的に生体網内系機能を抑制する物質RDS(Reticuloendothelial Depressing Substance)と, 心筋を直接的にそして特異的に抑制する物質MDF(Myocardial Depressant Factor)の産生がそれである.

Title : Relationship between reticuloendothelial functions and cyclic nucleotides in endotoxin shock - Reticuloendothelial activity as related to adrenal function -

Abstract : [Summary] : The blood pressure on administration of endotoxin is temporarily lowered and is recovered to the original state but it gradually moves to a shock state from the time after 2 hr. Concerning the transfer to this shock state new findings were introduced on the pharmacological relationship between the reticuloendothelial activity and cyclic nucleotides and the roles of the defense system in vivo and the adrenal secretion system. There is a strong possibility that the initial septic shock due to endotoxin is mediated by the vasoactive substances, i.e. mainly histamine and the like. Furthermore, it was considered that an release of histamine was regulated on this occasion depending on cyclic AMP. However, it was shown that the subsequently caused transfer to an irreversible shock state was correlated to a decrease in the reticuloendothelial activity and that this activity had the positive correlation with the ratio of cyclic AMP/cyclic GMP, not depending on single cyclic AMP. The ratio of cyclic AMP/cyclic GMP was significantly lowered by a treatment of adrenoectomy and further it remarkably fell on the administration of endotoxin. Also the granulopectic index (K) varied quite similarly to the ratio of cyclic AMP/cyclic GMP. The decrease in the granulopectic index (K) due to endotoxin administered to adrenoectomized rats was recoverd to a normal state on the administration of hydrocortisone succinate and this tendency toward the recovery shifted to the right side compared with the sham group. Moreover, as a result of administering hydrocortisone succinate (50-200mg/kg i.v.) to adrenoectomized rats with endotoxin shock, the survival time was prolonged 4-10 times. From the above it suggests that the defense system in vivo against endotoxemia depends on maintenance of the reticuloendothelial activity through the ratio of cyclic AMP/cyclic GMP due to adrenal cortex hormone.
