Abstract : 「抄録」 67Ga-citrateは, 悪性の腫瘍細胞に特異的に取り込まれるアイソトープではない. 炎症はじめ正常な肝臓, 骨, 唾液腺にも取り込まれ, 腫瘍のイメージ診断としては満足すべきものではない. 最近, 注射した67Gaは血液中で鉄のcarrierであるtransferrinに結合して, 腫瘍細胞に運ばれることがわかって以来, 67Gaと鉄との関係が注目されている. そこで, 今回, 血清鉄と67Gaの代謝の関係を検討し, より良い腫瘍イメージをとるために, (1)担癌マウスに放射線を全身照射して血清鉄値を変化させる. (2)担癌マウスに鉄剤のFesinを注射して血清鉄値を変化させる. という方法を用いて実験を行なった. その結果, つぎの結論をえた. 1)担癌マウスの血清鉄値が低いときは, 67Gaの腫瘍への取り込みおよび体内分布は増加した. このときの67Gaの排泄は遅延した. 2)担癌マウスの血清鉄値が高いときは, 67Gaの腫瘍への取り込みおよび体内分布は減少した. このときの67Gaの尿, 糞からの排泄は, 対照群のそれと逆転して尿から多く排泄し, かつ, 67Gaの排泄は促進された. 3)担癌マウスに67Gaをさきに注射して, その後Fesinを注射したときは, 腫瘍および各臓器の67Gaの取り込みは, やや減少する傾向にあった. しかし, それ以上に血液中の67Gaのclearanceが大きく, 腫瘍/血液比は高くなり良い腫瘍イメージをうることができた. 以上の結果は, 担癌家兎の腫瘍イメージの結果と一致し, 臨床への利用の可能性を示唆した.

Title : The Influence of Iron on Gallium-67 Accumulation in Animal Tumor

Abstract : Gallium-67 concentrates in a variety of soft-tissue and tumors. However, its accumulation is not specific for malignant tumor. Many normal tissues, particularly the liver, may accumulate apprecciable amounts of the 67Ga. The question of how the tumor to non-tumor ratio may be increased is important to improve the quality of the tumor image. Recently, it was found that the clearance of 67Ga from blood and the attainment of high tumor to normal soft-tissue concentration ratios are accelerated if the serum iron concentration is high. The present investigation was undertaken in order to study the relationships between the change in the serum iron concentration and 67Ga accumulation in the tumor tissue in mice bearing Ehrlich's ascited tumor. The following results were obtained. (1) The serum iron concentration was significantly decreased between 3 and 6 hours after 1000 rad dose of whole-body 60Co irradation. Subsquently, the serum iron levels were slowly elevated. (2) The uptake of 67Ga in the tumor and soft tissues was increased if the serum iron concentration was decreased by 60Co whole-body irradiation during the early phase. On the contrary, if the serum iron concentration was high, the uptake of 67Ga in the tumor was decreased. (3) The excretion of 67Ga from the body was delayed if the serum iron concentration was decreased by 60Co whole-body irradiation. However, if the serum iron concentration was high, the excretion of 67Ga from the body significantly increased. (4) The uptake of 67Ga in the tumor and soft-tissues was decreased compared with controls if the serum iron concentration was increased by Fesin(iron) administered prior to injection of 67Ga. (5) The excretion of 67Ga from the body was considerably increased if the serum iron concentration was increreased after Fesin administration prior to injection of 67Ga. (6) The accumulation of 67Ga in the tumor and soft-tissues was slightly decreased compared with controls after Fesin administration 3, 6 and 24 hours after injection of 67Ga. However, the tumor to blood ratio (T/B) in each group treated with Fesin (iron) was considerably increased because the clearance of 67Ga from the blood is accelerated. The large tumor to blood ratio (T/B) would be advantageous for tumor scanning. The results are consistent with the hypothesis that binding of 67Ga to transferrin is very important to the accumulation of 67Ga in tumor tissue.
