Abstract : 「まえがき」 歯の奇形の発現機構は発生中の器官原基の癒合や分裂とその原因をなす組織の崩壊によって, 結果的に器官の矮小化や欠如として現われる全身の奇形のそれと同じであろう. 歯にも癒合歯があり, 原基の分裂に関わりをもつ双生歯, 重複歯, 過剰歯がある. 歯の部分的な分裂異常としては過剰結節や過剰根がある. 癒合歯についての症例報告は多いが, その組織学的研究報告はきわめて少ない. 移植された2つの前臼歯胚の癒合例を除けば, 歯胚を切断したり, ばらばらにしたものを組み合わせて移植することによって臨床で見られるような癒合歯, 過剰根を作ったという報告はない. 多数の癒合歯の組織像からその奇形発現の機構を推定することが, この研究の目的である. 双生歯は一般に癒合歯として取扱われているが, 学者によっては分離不全のものではないかという説もあるので次の分類によって知見をまとめた.

Title : A Histologic Study of Fused Primary Front Teeth I. A Consideration on the Mechanism of Incidence of Tooth Fusion

Abstract : Extracted 16 cases of fused primary front teeth, i.e. 5 cases of 「AB, 4 cases of BA|, 2 cases of 「BC, 2 cases of CB| and 3 cases of geminate-like teeth 「C, C| and A|, which were studied macroscopically, roentogenographically and histologically. The mechanism of the incidence of tooth fusion was considered on the basis of histology. The following findings were obtained : 1) Fused enamel of the teeth contained frequently more numerous enamel lamellae and tufts, abnormally shaped parazones of enamel prisms, malformed or undercalcified enamel prisms, and some cell debris, 2) The thickness of the enamel on the fused side of teeth was generally decreased compared with that on the nonfused side, 3) Fused dentin of the teeth contained frequently lesser number of dentinal tubules and an appreciable number of cell debris. These findings suggest that the fusion of teeth is probably related to a pathologic condition which took place in the early developmental stages of the tooth bud and lasted for a relatively long period via the affected pregnant mother.
