Abstract : 「はじめに」 近年, RPD製作に際し, 術前, 術後のプラークコントロールに多少の注目がなされるようになってきている. RPD装着者の支台歯のう蝕罹患率は他の残存歯より高く, 床歯頸縁からの機械的刺激やプラークの付着, 外傷性咬合などから歯周疾患を誘発しやすいといわれている. 床歯頸縁が歯頸部を被覆あるいは接触していると, 残存歯の歯頸部と床歯頸縁の間隙が狭くなり, 食物残渣の停滞, プラークの付着などが著明になりやすく, 床歯頸縁が歯頸部から離開していると食物の流れがよく, 自浄作用が働きプラークの付着も少なくなりやすいと考えられる. そこでKarlsenとOwallらは床外形を改良することによって, 清掃効果を円滑にし, プラークの付着を少なくし自浄作用を期待した設計を試みている. 今回我々は補綴臨床において床歯頸縁の設計差と残存歯およびデンチャーに付着するプラークとの関連について実験をおこない, 多少の知見を得たので報告する.

Title : Gingival Margin Coverage Ratio of a Dental Base - Its Effect on Plaque Accumulation in the Remaining Teeth - A Study

Abstract : Abstract : In recent Years incidence of caries and periodontal disease due to plaque accumulation among wearers of the RPD has attracted attention. We conducted a study on the effect which difference in cervical margin specification of the denture base have on plaque accumulation in the remaining teeth. First, using 10 male students as a test group we constructed three types of the denture base : 1) covered type which covered the gingival margin of the teeth, 2) contact type which came into contact with the gingival margin, and 3) open type which left the gingival margin exposed after which we used the Green & Vermillion Oral Hygine Index to measure plaque accumulation a after periods of 1 day, 3 days, and 1 week. Since there is no existing standard index for prosthesis we used the above index as a casis for our own index to learn how differences in prosthesis type affect plaque accumulation. Our results were follows : 1) Plaque accumulation in remaining teeth was highest at 1.0 for the covered and contact types and lowest at 0.6 for the open type prosthesis. 2) Regardless of day of inspection, the covered and contact types showed higher plaque accumulation than that for the control group. 3) For plaque accumulation on the menbrane contact surface of a maxillary prosthesis there was significantly lower accumulation than that for the other points measured. 4) After 1 week the plaque accumulation on the basal surface for the open type was highest at 1.5.
