Abstract : 「緒言」 歯肉剥離掻爬術は歯周疾患の治療体系の一環として日常の臨床にしばしば行われており, その治癒機転, 治癒経過について従来多数の基礎的, 臨床的研究がなされている. 歯肉剥離掻爬術の創傷治癒に関する組織学的研究ではMorrisが上皮の再付着を報告し, 伊藤はヒトについて実験的歯肉剥離創の治癒過程を組織学的に観察し, 短期間にセメント質および象牙質に上皮の再付着をみたと報告し, 爾来, 臨床的あるいは実験的報告が数多く見られる. このような病理組織学的検索法は, 治癒経過を観察するうえで客観的かつ信頼性の高い的確な方法と考えられるが, 日常の臨床において歯周組織を切除し治癒経過を観察することはきわめて困難である. 近年, 歯周組織の代謝を反映していると思われる歯肉溝滲出液について多くの研究がなされ, これらは歯周組織の状態を間接的に知るための有用な手段とされている. 歯肉溝滲出液に関する生化学的ならびに生理学的研究の端緒はBrillならびにWaerhaugらによって開かれた.

Title : A Study of Protein Fraction Changes in Gingival Crevicular Fluid Following Flap Surgery for Patients with Periodontal Disease

Abstract : Abstract : For a thorough understanding of periodontal disease, gingival fluid has been studied and its composition and mechanism of exudation investigated. Gingival Crevicular Fluid (GCF) was found to contain a plasma protein but the changes in the protein component of the GCF have not been illucidated. This experiment was an attempt to determine the effects of flap surgery treatment upon 52 caees of periodontal disease using GCF as an indicator of wound healing and of the severity of the disease. To examine to protein concentration of GCF as a means of evaluating the severity of the periodontal disease and the wound healing after flap surgery, GCF was taken from gingival pockets of all patients, by strips of filter paper, before surgery and 1, 2, 3, 4, and 12 weeks after flap surgery. Quantitative analysis of protein fractions was performed by disc electrophoresis and was compared to various clinical findings such as: PMA-I, pocket depth, tooth mobility and pus discharge. Pre and post treatment values for protein concentration and clinical findings were compared. This study showed that the ratios of protein fractions in GCF could be classified into types and that these four types could be relatd to clinical findings and the severity of periodontal disease. It was found that the quantity of albumin from GCF of patients with periodontal disease was lower before flap surgery and increased significantly according to the process of healing in the wound after surgery. Thus it is suggested that the changes in the protein fractions may be used as a criterion for determining the severity of periodontal disease and the degree of wound healing after flap surgery.
