Abstract : 「緒言」 パーシャルデンチャー(RPD)は形態や機能を回復し, ならびに残存組織を障害せずに顎口腔系と調和し長期間保全しつづける事が重要な使命である. これらの調和が破壊される事によって補綴物の予後の良否が大きく左右され, なかでも遊離端義歯は人工臓器として生体に対する調和が困難とされている. これは支台歯および残存歯の変位量と床下組織, とくに顎堤粘膜の変位量との差が大きいためである. また, 咬合力や咀嚼力の負担は歯牙および粘膜による混合負担様式をとるが, 義歯後方に支台歯が求められず, 義歯が不安定となって残存組織が損傷されやすい. J. N. AndersonとG. A. LammieはRPD装着者の予後を調査し, 被検者507名中124例の下顎遊離端義歯の約半数が使用されていなかったと述べており, これは前述の条件の悪さから起因するものであろう. また, G. E. Carlsson, B. Hedegard, K. K. Koivumaaら, 尾花らも予後に関する調査による支台歯の変化, 義歯の安定性, 義歯床下粘膜の状態などについて経時的観察からみても中間義歯に対する遊離端義歯の困難性が理解できる.

Title : Displacement of Distal Extension Removable Partial Dentures as Observed with a Fiberscope

Abstract : Abstract : Distal extension partial dentures are said to be difficult to harmonize with the oral tissues when used as artificial appliances. This is due to differences in displacement of the abutment and remaining teeth when compared with that of the mucosa. As a result, the most important factor is the equal distribution of occlusal and masticatory forces in the remaining teeth and the edentulous alveolar ridge. In order to demonstrate this, the amount of displacement of the denture base from the teeth and alveolar mucosa during function was measured. In the present study, the author affixed the end of a fiberscope together with a core to the distal abutment tooth at a point near (10mm) the clasp tip developed by Matsuo et al in 1971. The fiberscope method was then used to directly observe the displacement of the denture in the oral cavity. By so doing, it was possible so determine the displacement of the distal extension base within the sagittal plane. The following results were obtained. 1. By using the fiberscope method it was possible to determine the displacement of the denture in the sagittal plane during mastication in terms of fixed quantities. 2. When the balancing side was observed, the mesiodistal displacement of the denture was characterized mainly by torque of the denture base along with movement in a distal direction. 3. When the working side was observed, the vertical displacement of the denture base ranged from 0.11 to 0.15 mm. The difference in the loading position had the greatest influence, although it was difficult to determine because of differences in foods or clasps used. 4. When the working side was observed, the mesiodistal displacement at the measurement point near the rest on the denture was between 0.07 and 0.15 mm in a mesial direction. The displacement at measurement point near the margin of the denture was between 0.25 and 0.30 mm, also in a mesial direction.
