Abstract : 「緒言」象牙粒は歯髄内に腫瘤状に増大した石灰化新生物で, その発生に関しては諸説紛々であり, 未だ意見の一致がみられない. Fischer & Landois (1909), Euler & Meyer (1927) は歯髄細胞から二次的に発生した象牙芽細胞が象牙粒の形成に関与するとしている. また, Munch (1925), Fridrichovsky (1933), 向野 (1933) は, 象牙質の形成異常によって生ずるとしている. さらに, Stenvik & Mjor (1970), Moss-Salentijn & Klyvert (1983) は発育中の歯根の機械的な圧入によって根髄部に上皮鞘の迷入を来たし, これが核となって象牙粒を生ずるとしている. 一方, Baume (1980) は, 新しい象牙質が形成される際のparaphysiologicな現象であると考えている. 象牙粒は解剖学的に遊離性, 壁着性および介在性に分類され, 組織学的には真性, 偽性に分類される. 大きさは微粒子状のものから髄室全体を埋めつくすものまであり, 時として神経線維を圧迫し特発性歯髄炎の原因となることもある.

Title : A Clinical Evaluation on the Denticle

Abstract : The denticle is a form of calcification found in dental pulp. The sizes are ranging from small microscopic particles to stones obliterating almost the entire pulp chamber, but the development process still remains obscure. Previous studies on the denticle were mainly based on anatomical and histological evaluations, and there were a few studies from a clinical aspect. In order to evaluate the effect of the denticle for root canal treatment, full-mouth radiographic surveys were conducted on patients of 458 females and 414 males ranging from 10s to 60s of age, who were chosen at random from the charts of Kanagawa Dental College Hospital. The incidence of denticles was evaluated in comparison of the sex, the age, the tooth group, and the condition of the tooth crown. In addition, a clinical study was performed on the horizontal section samples of 12 extracted maxillary first molars, which were diagnosed as contained the denticle by the X-ray films, and the pulp chamber and the orifices were observed before and after removing the denticles. The results obtained were as follows. 1) In spite of the sex, upper and lower tooth, and right and left side, the percentage of the denticles in molars was more than 20%, and that in premolars was less than 5%. This was statistically significant・ 2) Regarding to the ages, the percentage of the denticles showed a tendency to increase suddenly in ages of 20s, and the highest percentages were showed in 30s and 40s. 3) Concerning the condition of the tooth crown, the restored teeth indicated the highest percentage of the denticles. The tendency was indicated strongly in ages of 30s-50s. 4) 12 extracted maxillary first molars, which were diagnosed as contained the denticle by X-ray films, were confirmed as the denticle by the observations of the horizontal section samples. 5) The observations of the horizontal section samples showed that all orifices were covered with the denticles in some cases, and that the second mesio-buccal root canal was occaionally discovered after removing the denticles.
