Abstract : 「緒言」 歯肉内縁上皮は, 歯牙の全周を囲繞し, その機能は, 支持組織としての働きだけではなく, 内縁上皮下の血管網を介して, 滲出液の代謝などにも関連し, 特異な作用を営んでいる. 一方, 臨床では歯冠補綴及び有床義歯の辺縁部に接し, 内縁上皮とは深い関係を持っている. ことに, 日常, 補綴前処置としてよく行われる歯肉整形術や歯肉切除術などは, 直接, 内縁上皮に外科的浸襲を与えている. この内縁上皮は, 口腔側の外縁上皮と共に歯肉を形成し(Gottlieb), このうち, 外縁上皮は, 厚い角化層を有する重層扁平上皮で, 上皮細胞間は, 密に結合し, 粘膜固有層を介して, 歯槽骨に結合して, 食物の流路及び食塊形成などの機能の一部を荷っている. これに対し, 内縁上皮は, 非角化性の上皮で, 歯小皮を介して, 歯面と接着している付着上皮 (attached epithelium) と, その上縁は歯から遊離した溝 (gingival sulcus歯肉溝) に面した上皮とに区分される.

Title : Experimental investigation on vascular reconstruction beneath the inner ginival epithelium using corrosive resin cast

Abstract : The inner gingival epithelium consists of a non-keratinized epithelium, the lower part of which abuts on the tooth surface by dental cuticle and the upper marginal part encompasses the tooth, leading to the formation of gingival sulcus. The gingival sulcus is always moistened with fiuid which seeps from the inner gingival epithelium. It has been suggested that the vascular network beneath the inner gingival epithelium derives from that bistributed over the reduced enamel epitelium and that underlying the oral epithelium, both of which fused together when the tooth has penetrated into the oral cavity. A scanning electron microscopic study using corrosive resin cast has been carried out in an attempt to observe the morphological changes of vasculature beneath the inner gingival epitthelium after the reduced enamel epithelium, prior to eruption, has been excised, followed by the completiom of tooth eruption. And the effects of the dental calculus and chronic inflammation, as well as the operation technique on the morphological changes of vasculature also have been investigated. Unerupted mandibular first molar of four puppies have been used for developmental study and clinically healthy gingiva of fifteen dogs have been used for gingivectomy study. 1. In development study experiment, when the possible largest amount of the reduced enamel epithelium has been excised, fence-like pattern, which differed entirely from cross fence-like vascular network seen in the normally developed gingiva, could be visualized in the regenerated gingiva after tooth has erupted. 2. In the experiment in which gingiva has been excised in different directions from an abult dog, cross fence-like vascular network formation could be identified in all specimens when a part of cross fence-like vascular network was left beneath the inner gingival epithelium, and the fence-like vascular network was formed from the oral epithelium side. 3. The regeneration of fence-like pattern vascular network beneath the oral mucosal epithelium was earlier than that of cross-fence type beneath the inner gingival epithelium. 4. When the vascular network was accompanied with inflammation, not only the expansion and tortuosity of capillary and extravasation of resin but also the glomerular type of vascular loops could be identified. 5. It is indicated that the formation of glomerular type vascular loops might be a result of three dimensional change; that is, due to the compression of a part of inner gingival epihelium into a deeper portion of gingiva, leading to a compression of cross fence vascular network beneath it, thereby glomerular type vascular loops formed.
