| Abstract ： | 「緒言」上顎前突症は矯正治療を希望して来院する患者の中で下顎前突症, 叢生についで頻度の高い不正咬合の1つである. いわゆる上顎前突症は上下顎歯列弓の近遠心関係の異常で, 上歯列弓に対して下歯列弓が遠心に咬合するものをいい, Angleの分類ではII級1類に相当する不正咬合である. この中には, 上歯列弓の位置は正常では下顎のそれが遠心位をとるもののほかに, 下歯列弓は正常で上顎のそれが近心位をとるものの存在が頭部X線規格写真によって明らかにされている. また, JarabakはII級1類不正咬合を 1)歯および歯槽性II級 2)機能的II級 3)骨格型II級 4)複合型II級の4型に分類し診断および治療の指針の1つとしている. II級1類の矯正治療にあたって特に慎重な考慮を要求されているのは, 治療を抜歯によっておこなうか, または非抜歯でおこなうかの鑑別である. 一般的には前歯の被蓋の深い症例で顔面の成長方向が水平的である場合は非抜歯とすることが多く, これに対して被蓋が比較的浅い垂直的成長を示す例では第1小臼歯の抜去をおこなって治療を進めるのが普通である. |

| Abstract ： | A class II malocclusion case accompanied with deep over bite and discrepancy between tooth size and basal bone was treated orthodontically after extraction of four second molars. Visual examination showed that this patient had a potential of mandibular forward growth, indicating the decrease anterior facial height which was undesirable to reduce the anterior overbite. The patient was treated in strict accord with the outline of treatment for a non-extraction class II malocclusion case described in elswhere. The tooth movements were principally the uprightig and retraction of molars, the retraction of maxillary anterior teeth and the consolidation of spaces. The chages in facial growth were quiet, but in a downward direction. These changes contribute to the opening of vertical dimension. Two things must be considered in the treatment of class II malocclusion; decrease in deep overbite and a lingual over-retraction of the maxillary anterior teeth. Extraction of maxillary second molars in the treatment of class II malocclusion is done under the following conditions; the growth pattern is counterclockwise, there is a deep overbite, the thired molars are present and their roots are begining to be formed. Treatment of malocclusion such as the one described in this report should be done with extraction of second molars. |
