Abstract : 「緒言」体性神経刺激による自律神経系への反射は各種報告されているが, 特に三叉神経を入力とした自律系への反射も多く存在することは, 日常の歯科臨床でしばしば経験するところである. 口腔領域は脳神経が密に分布しているので, 歯科治療時の患者の肉体的, 心理的負担は大きく, 口腔内の疼痛刺激が引金となる全身異常は高頻度に見られる. 刺激に対する生体反応の発現機構に関してReilly (1934) は "中枢, 末梢を問わず自律神経系の一部に強度の刺激あるいは, 弱くても持続的な刺激が作用すると, 病的な自律神経反射をひきおこし, しかも病変の出現する部位は刺激の加わった自律神経に直接関連した近接の臓器のみでなく, 刺激部位とは全く無関係と思われる遠隔の臓器にまで器質的病変を誘発する" とした, いわゆる自律神経過剰刺激症候群 (Reilly現象) を提唱した. 典型的なReilly現象発現時には, 副腎, 肺, 心, 胃, 腸の全身諸臓器の出血性の病変が見られる.

Title : Effects of Trigeminal Stimulation on the Solitarius Nucleus Neurons in Cat

Abstract : [Abstract] : The Reilly syndrome is considered to be experimentally or clinically induced disturbances in different organs mainly consisting of microcirculatory disorders due to the irritation of the autonomic nervous system, either in the peripheral or central portions. In cases of irritation of the somatic nerves in a specific area such as the oral cavity and pharynx where the sensory fibers are abundant, the same type of pathological changes as the Reilly syndrome occur. To elucidate the mechanism of the Reilly syndrome in oral medicine, the functional relationship between trigeminal input and the function of the solitarius nucleus neurons was examined. Cats of both sexes were used. The discharge pattern changes of nucleus solitarius neurons were examined when the infraorbital and mandibular nerves were stimulated. The trigeminal nerve input had three types of effects on the solitarius nucleus neurons, i.e., excitatory, inhibitory and indifferent. The distribution of neurons of these three types was random. The above findings suggest that the trigeminal stimulation has an influence on the vagus nucleus neurons, and this may be one of the causes of the Reilly syndrome in oral medicine.
