Abstract : 「はじめに」1歳6ヵ月児歯科健康診査 (以下1.5歳児健診とする) は, 昭和52年6月の厚生省児童家庭局長通知に基づき, 同年10月から補助金交付による特別区ならびに市町村の事業として開始されたものである. 本健診の目的の1つに, 乳歯う蝕がまだ多く発生していない時期に将来のう蝕発生を予測し, それに応じた適切な保健指導を行うことがある. すなわち, 1.5歳児健診を利用して乳歯う蝕の発生と進行を抑制しようというわけである. そのために, O1・O2・A・B・C型というう蝕罹患型が設定され, 歯科的な生活状態に関する問診や歯の汚れに関する診査も行うことになっている. しかしながら, 実際には, 健診日当日の口腔衛生指導やう蝕部に対するフッ化ジアンミン銀製剤の塗布だけで終わることが多く, その後のフォロー・アップを実施しているところは少ないようである. そのためか, 1.5歳児時点のO1・O2型における予測性と3歳児時点の乳歯う蝕の多少との間に相関が見られるというような, しごく当然で, したがって本来の意味においては健診後の管理不十分と評価されるべき報告が多い.

Title : An Approach to the Planning of Community Health Programs for Dental Caries in Deciduous Teeth at the Early Childhood. - Screening through the Oral Conditions of Children Aged One and a Half. -

Abstract : [Abstract] : Most of primary teeth are attacked with dental caries at the age of one and a half to five. From the public health point of view, yet, the strategy for caries prevention of primary dentition has not been established. In the present study, the relationships between the dental caries of primary dentition and the dental health behavior at the early childhood were investigated. First, 166 children aged 1.5 years were studied in 1980 (April to December), and then they were reinvestigated when they became 5 years old in 1984. The results were as follows : - Concerning the dft index at the age of 5 years, in the type O2 (at the age of 1.5, nothing carious teeth but with the higher risk of caries attack), a significantly higher score was obserbed than in the type O1 (at the age of 1.5, nothing carious teeth and with lower risk of caries attack). It was considered that the classification of carious types in primary dentition at the age of 1.5 was useful for screening in estimating the condition of dental caries at the age of 5. However, it is difficult to change the individual dental health behavior by dental health instruction only, at the age of 1.5. Therefore, the successive dental health administration system in the public health level from 1.5 to 5 years old should be established, for caries prevention of primary dentition. In addition, the topical fluoride application at the age of 1.5 should be recommended.
