Abstract : 「緒言」補綴物を口腔内に装着する目的は, 喪失した顎口腔系の形態と機能の回復ならびに残存組織の保全にある. その目的を達成するためには, 補綴物が生体組織に対し為害作用をおよぼさないことが重要である. したがって, すべての補綴物には生物学的所要条件を満たすことが要求され, 唇顎口蓋裂患者の永久保定装置についても例外ではない. 唇顎口蓋裂患者の補綴治療に関しては, 現在まで多くの症例が報告されている. その中で, 初期の症例においては外科治療に続く処置として補綴治療が実施されている. その処置内容は, 一般に二重歯列部分床義歯すなわち天然歯の外側に人工歯を排列したものとし, 維持力をクラスプやアタッチメントに求めていた. しかし, これらの処置方法では天然歯間における嵌合状態は得られず, 咀嚼力が比較的劣るといった面が存在していた. それに加え, 舌房が狭くなるため発音障害を引き起しやすくなること, 床内面が不潔になることなども問題点となっていた.

Title : An Experimental Study of the Palatal Mucosa Reaction to a Reinforced Bar used as a Permanent Splint for Cleft Lip and Palate Patients

Abstract : [Abstract]: An experimental study was conducted to investigate the reaction of the palatal mucosa to a reinforced bar used as a permanent splint for cleft lip and palate patients. The following materials were used fabricating the reinforced bars : (1) 12% Au-Ag-Pd alloy, (2) 1% Pt-Au alloy, (3) Porcelain fused to metal. Six of each bar type were placed into 18 subject mouths for a 30 day period. After removal, the bar contact to the palatal mucosa was observed macroscopically. The palatal tissue surface was also observed using oral exfoliative cytology and under the oralmicroscope. The following results were obtained. 1. Regardless of the bar composition, plaque was present in the vicinity of the bar contact to the palatal mucosa. However, quantitatively plaque levels were remarkably less in the porcelain fused to metal situation. 2. The individual epithelial cell was observed in all subjects. In all subjects, except those with the porcelain fused to metal bars, it was observed that the orange-red non-nucleated cells were less on the decrease and the orange-red nucleated cells were on the increase after placement of the reinforced bars into the mouths. In the porcelain fused to metal subjects, there was little change in the appearance of the orange-red nucleated cells. 3. Capillary loop changes were observed in the subjects with reinforced metal bars but were absent in the porcelain fused to metal situation. 4. The surface characteristics of normal epithelial tissue were present with the porcelain fused to metal bar but changes such as irregular shapes and the presence of nucleated cells were noted with the metal bars. Those results suggest that the porcelain fused to metal bar as a permanent splint for cleft lip and palate patients will react less to the palatal mucosa.
