Abstract : 「緒言」歯の動揺は, 歯周組織の炎症や歯槽骨の吸収, 咬合異常等により支持組織に損傷をうけた場合に起こる重要な臨床症状の一つである. 古くから歯科各分野において動揺度測定の研究が進められているが, 補綴学においても支台歯や鈎歯の動揺度測定は補綴物の設計, 装着後の予後観察に大きな影響を与える因子である. しかし現在, 一般臨床において行われる歯の動揺度の診査は, 視診, 触診, 打診, X線診査を基にMillerの測定法によって手指, ピンセットで歯を動かして行われるもので, 術者の臨床経験などの個人差によって左右される不確定な要素があることは否定し難い. 歯周組織の性状を客観的に評価しようとする試みは, 古くから行われ, これらの測定法には種々あるが歯に外力を与えてその変位量を測定した, ダイヤルゲージ法やストレインゲージ法から端を発していると思われる. その後, 倉島, parfittらの差動トランス法を利用した電気的測定法を経て, 近年のME機器の発達とともに, 歯及び歯周組織に与える外力を衝撃としてとらえる考え方が多くなってきている.

Title : THE Mobility of Healthy Teeth as Measured with the Impact Hammer Method

Abstract : [Abstract]: Determining the extent of tooth mobility is an important factor when designing prostheses. However, examination in an actual clinical setting requires methods such as palpation, percussion, X-rays film and use of Miller's technique, all of which require moving the teeth with the fingers or tweezers. This leads to inaccurate results since measurements are effected by the clinical experience of the individual practitioner. A battery of acoustical-vibrational analyses was conducted in an effort to solve these problems. Results of analysis show that it is possible to determine the the rate of tooth mobility from the length of the primary wavelength on a time domain when a microphone attached to an impact hammer is used in order to record impact sound. In the present study, the author made use of an impact hammer developed at this laboratory laboratory as well as a 2 ch FFT to measure impact on 100 normal test subjects under a number of conditions. By performing these experiments the following results were obtained. 1. The primary wavelength for the normal teeth was between 1.72 and 2.19 msec. 2. Differences were recognized in the value for the primary wavelength depending on such factors as tooth type, direction of impacs and the sex of the patient. 3. Single tooth restoration did not show a significant difference when compared with natural teeth. 4. In general, the value for canines was the smallest and values wgre generally greater for males than for females. The significant difference for incisal edge and occlusal percussion was smaller than that for lingual and labial percussion under conditions including the type of tooth, sex of the subject and whether or not there had been dental treatment.
