| Abstract ： | 「緒言」口腔外科手術は歯科治療のなかで, 疼痛を伴う観血的処置が多く, 症例によっては手術時間も長時間に及ぶものがある. このことから, 治療に対する患者の不安感, 恐怖感などの精神的ストレスは, きわめて大きいことが推測される. 精神鎮静法は, 患者の精神的ストレスを薬物を用いて軽減し, さらに和痛効果と健忘効果を期待できる方法である. しかも精神鎮静法においては, 諸反射が保たれており, 生理的状態はほとんど変化しないため極めて安全にして優れた方法である. 精神鎮静法には, 笑気などを用いる吸入鎮静法と, 緩和精神安定剤, バルビタール剤あるいは麻薬などを投与する静脈内鎮静法がある. このうち静脈内鎮静法は鎮静効果が確実であることから, 吸入鎮静法が適応できない症例にも有用性か高い. ベンゾジアゼピン誘導体のフルニトラゼパムは, 1971年Vegaによって初めて麻酔導入薬として使用されて以来, 歯科領域でも幅広い使用が試みられている. |

| Abstract ： | [Abstcact]: Sedative effects of flunitrazepam 0.015 mg/kg I.V. were studied in 30 patients undergoing oral surgery under local anesthesia. Following results were obtained. Verbal responsiveness was slightly depressed. Dysarthria was found in 26 patients (86.7%) out of 30, especially remarkable immediately after administration. Mean blood pressure, pulse rate and respiration rate were remained almost unaltered. A slight supralaryngeal obstruction was observed in a case by the relaxed tongue after administration with flunitrazepam. All the cases could be allowed to discharge at 120 mintues afer I.V.. One patient complained of a pain at injection, but not any other venous complication was observed. It was concluded that inravenous sedation with flunitrazepam was effective and available for outpatients of oral surgery with local anesthesia. |
