Abstract : 「緒言」前歯用の審美性修復材として発展してきたコンポジットレジンの臼歯咬合面部に対する適用が試みられて以来すでに久しい. 当初, 臼歯用コンポジットレジンの機械的特性はアマルガムのそれに匹敵し, 耐摩耗性も優れているとの報告もみられ, その臨床的有用性が期待された. しかしながら, それら臼歯用材料の口腔内装用試験については, 必ずしも満足すべき結果が得られないとの報告も散見されたため, 著者らの教室においても本材料の比較的初期の二, 三の製品について, 一般物性試験の他に疲労試験を行い, 同時に短期間の臨床的観察を試行した. その結果, 供試した臼歯用コンポジットレジンはいずれも, 6ヵ月迄の間に修復物辺縁部に小破折を生じること, さらに引っ張り, 曲げ, および疲労強さと修復物辺縁の劣化には1ないし3ヵ月の観察結果において相関がみられ, とくに, 疲労強さとの間には高い相関が示されたことを前報で報告した. 今回はさらに症例数を増すとともに, 前報における症例中リコールに応じたものを含め, 修復物の3年間にわたる予後を追跡調査して若干の知見を得たので報告する.

Title : Three-Year Clinical Results of Posterior Composite Resins - with Special Reference to the Marginal Degradation of Restorations -

Abstract : Abstract : In a previous study, a high correlation was found to exist between the data obtained from laboratory fatigue strength test and the durability of marginal integrity of posterior composite resin restoration aged six months in vivo. In this study, a further long term clinical evaluation for various posterior composite resins was carried out to determine the marginal degradation of these restoration over a three year period. One hundred and thirteen restorations were placed using with four different posterior composite resins (listed in Table 1), and conventional composite resin and high copper amalgam alloy. Patients were recalled at six month, 1, 2, and 3 year intervals after the restorations were completed, and the thorough clinical examination and duplication of the restored tooth surfaces were performed at each examination. Replicas were observed by SEM and photographed, and the marginal integrity were evaluated by five examiners. The findings were as follows : 1) The percentage of recalled patients was decreased as the time increased, only 43.4% of the original patients were complied with the testing requirements. 2) Although the degradation of marginal integrity of restorations were demonstrated in all tested materials, a slight difference of tendencies could be found. 3) No significant differences were shown in regard to the marginal design of preparations used and location of restored teeth.
