Abstract : 「緒言」ヒト顎骨骨髄における悪性腫瘍細胞の増殖蔓延状況についての報告は今日まで非常に多く, 実に枚挙にいとまがない, その大多数のものは臨床的症例報告かまたは摘出材料のごぐ一部についての検索であって, その内容は決して十分とは言い難い. 一方病理学的に顎骨の広範囲にわたる系統的組織学的研究は意外にも少なく, 僅かに教室の橋本, 山崎らによってなされているにすぎない. 著者は日頃下顎を1つの臓器と考える立場より, 下顎は歯の支持組織であるのみでなく, 造血組織をも含んでおり, 組織構築の上からもまた病変の進展からも両者には密接な関係がある. かかる視点より, 下顎骨骨髄, 歯槽部骨髄, 歯周組織および歯髄における悪性腫瘍細胞が, どの様に浸潤増殖し, その増殖様式にどの様な特長を有するものかを知ること, さらに腫瘍を骨髄の原発性と続発性に分けてみた場合, 如何なる差異がみられるかを検討した. そしてこれらの悪性腫瘍細胞の増殖が歯牙, 歯周組織および骨組織へ如何なる影響を与えるか, また治療の影響などについて解明することを目的とした.

Title : Spreading of Malignant Tumor Cells in Human Mandible - Histopathological Study of Autopsy Cases -

Abstract : Abstract : Based on large celloidin-embedded sections of 91 right mandibles from autopsy cases with malignancy, a histological study was performed to determine characteristics of mandibular involvement of various malignancies. Mandibular involvement was observed in 39 (43%) of the 91 cases, including 8 cases with multiple myeloma, 9 with leukemia, 10 with malignant lymphoma, 2 with neuroblastoma, and 10 with extra-oral carcinoma. Mandibular involvement was much oftener in hematological malignancies and neuroblastoma than in carcinoma. Of carcinomas, gastric and pulmonary carcinoma showed comparatively frequent involvement compared to other carcinomas. Total replacement of the marrow by tumor cells was constantly observed in neuroblastoma, while this phenomenon was rare in malignant lymphoma and sometimes observed in other malignancies. The proliferation pattern of tumor cells was mainly diffuse in hematological malignancies except for multiple myeloma with nodular as well as diffuse patterns, while carcinoma always showed massive pattern. In early stage of mandibular involvement, tumor cells of most malignancies revealed a distribution pattern similar to pre-existing hemopoietic marrow ranging from condylar process down to posterior part of the mandibular body. This indicated an important role of hemopoietic marrow for the early deposit of tumor cells. As the stage advanced, the tumor cells seemed to spread to anterior part of the mandibular body and to periodontal marrow, resulting in total replacement of the mandibular marrow. Involvement of periodontal tissues, especially gingival involvement, was oftener in hematological malignancies compared to carcinoma. Besides, in some hematological malignancies, gingival involvement seemed to occur independently from underlying proliferation of tumor cells in the mandibular marrow.
