Abstract : 「はじめに」 外科的療法は腫瘍組織および腫瘍転移巣らの摘出が困難な場合が多く, とくに顎口腔領域ではその全摘出が出来えがたく, 手術に随伴する因子, 器械的操作による腫瘍細胞の播種, 組織の増殖や促進物質の分泌亢進, 手術侵襲による生体の抵抗性減弱などが原因となって逆に腫瘍の発育をうながす. 摘出切除可能な場合でも, 血液中に腫瘍細胞の出現がみられるようである. 全身的にリンパ・血行性に撒布されうるし, あるいは循環せられた癌細胞そして局所的腫瘍細胞の増殖と発育抑制を考えると制癌化学療法剤の使用による効果が必要とされる. Kloppらにより動脈内歇的投与法ならびに持続的投与法と大量衝激投与法(動注法)らと悪性腫瘍に対する制癌化学療法らの併用療法が内外各科領域で研究がなされてめざましい進歩・発展をみている. 動注法は, 腫瘍の比較的近い支配領域に制癌化学薬剤を作用させてほかの臓器への流出による影響を最小限度にくいとめ, 局所に最大の効果を期待せしめると同時に血流中の癌細胞らと血管内の転移やリンパ節転移および癌細胞の撒布による阻止の効果を期待する方法である.

Abstract : [Abstract] Effects on the vascular wall of surgical chemotherapy against oral cancer. Regional perfusion in the jaworal cavity region in our clinic resulted in the leakage of the whole amount of the infused drug in a short time. The perfusion is accompanied by a drawback that it requires extracorporeal bypass equipment, while regional intra-arterial infusion is relatively simple in procedure. The patient of malignant tumor is ascendingly cannulated into the regional artery via an arterial branch or descendingly cannulated directly into the artery of the target region, and infused with a carcinostatic agent. However, because the examination of the cannulated arteries for intra-arterial infusion of carcinostatics revealed the presence of the effects of the drugs on them, experiments on animals were made to investigate the effects of various drugs on the vascular wall, and as a result, it was found that the vascular wall was affected with the lapse of time. The sites where the P・H tube was inserted, the sites where the tip of a cannula was located, and the circumferential blood vessels were clinically examined, and it was revealed that the effects of the drugs varied considerably largely with the location of the P・H tube and the state of the vascular wall of each patient. It was thus shown that because this treatment would involve the effects of carcinostatics on the vascular wall, and also because cancer patients consist of many patients of relatively advanced age, it would be nacessary to very carefully select a suitable carcinostatic agent for this treatment.
