Abstract : 「はじめに」 わが国の子供たちのう蝕状況が漸く改善し始めたとはいえ, 世界の先進国の中にあってはいまだに悪い状況にある. 1989年のWHO報告にみられるように, 世界の主要国における12歳児のう蝕状況を5段階に分けると, 日本は悪い方から2番目に属する, という不名誉な評価を受けた. このように, わが国の子供たちは全体的に高いう蝕状況を示しているのであるが, 細かくみれば, この中には極端に高いう蝕経験を示す少数の子供たちが存在していることも事実である. このような子供たちは, 集団のう蝕有病率に不均衡な影響を与えているばかりでなく, 生涯を通してのメインテナンスケアのニードも当然高くなってくる. これが"蓄積性の疾患"であるう蝕の弱点であり, "予防に優る治療なし"といわれる所以でもある. 一方, 子供たちのう蝕経験が全体的に低下し, う蝕はもはや子供たちに普通にみられる疾患とはいえなくなったアメリカにおいてさえも, "総う蝕経験歯数の60%はわずか20%の子供においてみられる"という不均衡な傾向にあるという.

Title : Studies on Caries Risk Assessment in a School Dental Health Administration I. Criterion for Identification of a High Caries Former in the Grade Sixth School Children

Abstract : [Abstract] Although there has been a little improvement in the dental health of Japanese school children, some children with high caries-experience still remain. It is important to predict a high caries former and to start preventive care for dental caries incidence in the permanent dentition as early as possible. The present study was conducted to make clear the definition of a high caries former and confirm the methodology to select him/her in the sixth grade. The subjects used in the present study were 1,085 school children who entered a certain elementary school in 1965 to 1979. They were divided into three groups according to the year of entrance: group A comprises 429 children who entered in 1965 to 1969, group B includes 345 children who are registered in 1970 to 1974 and group C is consist of 311 children who are enrolled in 1975 to 1979. First of all, a high caries former was defined by the criterion that is the children who are corresponding to twenty five percent of higher ranking of cumulative DMFT increment from the first grade to the sixth grade. When this criterion is used as the screening value of a high caries former, the DMFT increment isn't integral numbers. As screening values except integral numbers aren't of practical use, we managed to revise the screening values to integral numbers. Besides, it's to be desired that the numbers screened as a high caries former are about ten percent of subjects. From these points of view, it was concluded that the screening values as DMFT increment to select a high caries former wele >= 6 in group A (both male and female), group B (female) and group C (female), and were >= 5 in group B (male) and group C (male).
