| Abstract ： | 「緒言」歯科矯正治療においては, 歯科分野の中でとくに, 顎顔面頭蓋の成長を考慮せねばならない機会が多い. すなわち, 歯科矯正学は, 成長発育の様相を把握することによって, はじめて的確な診断や治療方針の決定, さらに治療計画の立案および予後の判定が可能となり, また, 安定な正常咬合を完成することができる, という性格を含んでいる. 近年, 予防矯正の観点から, 早期治療をおこなう傾向にあるものの, 成長発育の終了した成人の矯正治療が盛んに行われている. この傾向は, 積極的に推進したアメリカにはじまり, 全世界にひろまっている. しかし, この考え方は, 成人の矯正治療において, 安定した正常咬合を完成しようとする場合, 思春期性成長が終了した後もなお残存している顎顔面頭蓋における成長の潜在能力growth potentialityが, しばしば矯正治療中あるいは術後経過の観察中に, 治療目標に対してマイナスの要因として働くことがある. |

| Abstract ： | Abstract: It is important to understand the late growth of craniofacial complex after termination of adolescent growth in order not only to predict the changes of post-orthodontic occlusion but also to make proper orthodontic diagnosis for the adult patients. However, whether the late growth of craniofacial complex depends upon the somatic growth potentiality or functional deviation of occlusion due to eruption of third molars is not elear. Longitudinal cephalometric study using sixty-nine of Japanese subjects, 54 male and 15 female aged approximately 19 years through 23 years, has been done to examine the relationship between the growth of craniofacial complex and the increment of body height as an indicator of somatic growth and situational change of third molars as an indicator of local occlusal factor. Results indicated that growth of the craniofacial complex closely related to the occlusion of third molars and somatic height increasing. Especially, increment of ramus height (Cd-Go) significantly related to the occlusion of the upper and lower third molars, indicating that the functional occlusal change due to eruption of third molars influences vertical dimension of mandibular posture. From these findings, it was concluded that the orthodontic management of the normal eruption of third molar in adult patients is of particular importance in relation to late growth of craniofacial complex. |
