Abstract : 「緒言」根尖性歯周疾患は, 細菌学的および理化学的刺激が原因で成立すると考えられ, これらの原因の中でも, とくに根管経由で直接根尖歯周組織に対する刺激となる細菌由来物質ならびに組織応答物質の相互作用により病変が起きる. このような炎症刺激によって生ずる根尖性歯周疾患についての病因, ならびにその形成機序については古くから多数の報告がなされている. すなわち, 実験病理学的な根尖病変, X線学的な臨床例の検索など数多くの研究が蓄積されて来た. しかもこれらの多様な病変は, 軟組織および硬組織から構成される根尖歯周組織に生じ, とくに病因となる刺激の量と性質, 時間的経過とともに個体の抵抗力などが複雑にからみあって, 急性型や慢性型などの進展経過をたどり, X線的に確認される根尖病変の半数以上が歯根肉芽腫であるといわれている. そして, このような病変の処置として, 一般には根尖病変を惹起させる根管内の刺激物質を十分に清掃除去した後, 適正な根管充填を施すことにより多くのものは治癒に向うとされてきた.

Title : Effect of Root Canal Treatment on Cell-biologic Activity of Fibroblast-like Cells from Radicular Granuloma

Abstract : Abstract: The purpose of this study was to determine the effect of root canal therapy on the biologic activity of fibroblasts from periradicular granulomas. Access preparations were performed on mandibular premolars of dogs. Periradicular granulomas were produced by exposing the pulpal tissue to oral contaminantes for one month. Root canal therapy was performed on teeth of the experimental group. The teeth in the control group were not treated. At the end of one month, the dogs were sacrificed and specimens of the periradicular tissue were obtained. The tissue was culture in D-MEM supplement with penicillin and streptomycin. Individual cell cultures were prepared from each specimen. The cell cultures were then tested for Alkaline phosphatase and collagenase activity and specimens were also examined microscopically. The results obtained were as follows: 1. Microscopic evaluation of the tissue from both the control and experimental groups were consistent with periradicular granulomas. 2. Histopathological evaluation of the experimental group revealed no apparent healing of the granulomas in one month. 3. Analysis of the cell cultures indicated that the fibroblast-like cells were from the periradicular granulomas. 4. The histological appearance of the fibroblast-like cells of the experiments were similar with those of control groups. 5. The Alkaline phosphatase activity of the cells in the experimental group were significantly higher than the control group. 6. The fibroblast-like cells of both the control and experimental groups did not demonstrate collagenase activity. However, direct samples from periradicular granulomas demonstrated high collagenase activity. The results suggested that fibroblasts of periradicular granulomas do not participate in localized bone destruction without stimulus from inflammatory cells. Additionally fibroblasts may create an environment for bone remineralization in periradicular lesions within one month after root canal therapy is completed.
