Abstract : 「緒言」口腔癌の集学的治療において, 放射線治療および化学療法の併用は口腔の形態と機能を保全し, 治療成績を上げるうえで, 重要な役割を担っている. しかし, それらの治療に対する反応態度は症例によって, さまざまであり治療予後又は治療結果を予知, 判断する客観的指標が求められている. 近年, 腫瘍の細胞動態は放射線, 化学療法の感受性を左右する因子として注目され, 種々の細胞動態解析が臨床に試みられている. なかでも, 最近の細胞測光法の進歩により, 核DNA量の測定法が確立され, これを用いた細胞動態解析法が普及されつつある. しかし, 本法では同じ核DNA量をもつG0期とG1期の識別が不可能で, 放射線や化学療法の効果を左右する増殖相と非増殖相の動向を把握できない. この点に対し, 水谷, 木下はG1期からG2期まで合成を続けるRNAをDNAと同時に測光定量することにより, 口腔扁平上皮癌の細胞動態解析を行い, 核DNA-RNA量サイトグラムのパターンが治療効果と密接に関連すること, また治療過程における核DNA量とRNA量の量的変化が治療効果を反映することを明らかにした.

Title : Changes in the Nuclear DNA - RNA Contents and in the Cytological Feature During Therapy in Oral Squamous Cell Carcinoma

Abstract : Abstract : Studies were conducted on the treatment of oral squamous cell carcinoma and the relationship between the cell kinetics and the cytological feature, and the therapeutic effects of the process of radiation and chemotherapy. The study was conducted on 8 primary cases with oral squamous cell carcinoma. The cell kinetics were analyzed using polyphasic microfluorometric measurement of the nuclear DNA-RNA contents. The cell image of 1599 tumor cells were directly compared with the nuclear DNA-RNA contents. The following results were obtained. 1. Cell kinetics analysis of oral squamous cell carcinoma by DNA-RNA cytogram prior to treatment revealed 12%-36% in the G0/resting phase, 28%-49% in the G1 phase, 23%-34% in the S phase and 5%-32% in the G2 M phase and abortive mitotic cells. The clinical effect was better with fewer cells in G0/resting phase cells and more cells in G2 M phase and abortive mitotic cells. 2. Changes in the nuclear DNA-RNA contents and the cytological feature during radiation and chemotherapy increased, nucleus enlargement, aggregated chromatin, and pyknosis of the nucleus occurring more frequently with increased nuclear DNA contents. As the RNA contents increased, the frequency of cell enlargement, enlarged nucleolus, and vacuolated cytoplasm also increased. The cytological feature changed with the cell kinetics. Few cytological changes were noted in cells in the G0/resting phase cells, but most of the cells in the G2 M phase and abortive mitotic cells revealed nuclear, nucleolar, and cytoplasmic enlargement. The cell kinetics of oral squamous cell carcinoma appear to be related to the effects of radiation and chemotherapy, while the quantitative changes of nuclear DNA-RNA contents are closely related to the cytological feature. In particular, the abnormal increase in the nuclear DNA and RNA contents appeared to predict cell degeneration and death, and may be of clinical significance as an objective index for the assessment of the effects of radiation and chemotherapy.
