Abstract : 「はじめに」健全な乳歯歯列および咬合の保持は, 顎の正常な発育の上から重要な課題である. 近年乳歯齲蝕の減少の傾向がみられるとはいえ, 依然乳歯の齲蝕の罹患率は高い水準にある. しかも乳歯はエナメル質が菲薄であり, また象牙質の石灰化度が低いなどの特質を持つことより, 齲蝕の進行速度がきわめて速く容易に齲蝕が歯髄までおよぶこととなる. Eichenbaumは511人の小児のなかで213人は乳歯の齲蝕による露髄があったと報告している. このような乳歯に対しては何等かの歯髄処置が必要となるが, 歯根歯髄を残す生活歯髄切断法は, 乳歯根の生理的吸収過程を阻害しないという観点から, 小児歯科領域で最も有用な歯髄処置として広く用いられてきた. 従来から乳歯の生活歯髄切断法には水酸化カルシウム法とFC法の2法が臨床的に応用されてきた. これら乳歯生活歯髄切断法の臨床成績に関する報告はいくつかみられるがその評価は様々である.

Abstract : Abstract: This study was conducted to evaluate the prognosis (loug term results) of pulpotomized primary teeth at (in) the Pediatric dentistry clinic of Kanagawa dental college. The material consisted of 595 primary lower molers that had been subjected to pulpotomy with the calcium hydroxide technique or the formocresol technique between September 1982 and September 1989. 235 teeth of them were treated with calcium hydroxide technique and 360 teeth were treated with formcresol technique. The crieria for judging the results of these teeth was based upon the clinical records of each teeth, that is, teeth which were mainteined normal function untill the stage of natural shedding (exfoliation) considered to be successfull and teeth which were retreated or extracted before the stage of natural shedding (exfoliation) considered failure. The results were as follows; 1. Of 595 primary molers that have been pulpotomized, 489 teeth (82.2%) were considered to be successful. (The overell success rate of pulpotomy in 595 cases was 82.2%) In the case which were treated with calcium hydroxide technique, 198 out of 235 teeth (84.3%) were successful, although in the formocresol technique cases, 291 out of 360 teeth (80.8%) were successful. These difference was not statistically significant. 【There was no statistically difference betwee the success rate of calcium hydroxide technique and formocresol technique.】2. Age of patients at the time of pulpotomy distributed from 2 years old to 8 years old, and these success rates were 88.2%, 77.6%, 78.5%, 78.6%, 90.8%, 96.8%, 100%, respectively 3. The rate of failure was the highest during the period from 0 to one year following treatment and the rate of failure decreased as the time elapsed after treatment. 4. About 80% of failure cases were retreated by endodontic therapy and maintain the normal function untill the stage of natural shedding. Particurary, almost of all teeth juged as failure in early sage after treatment were mainteined by root canal treatment.
