Abstract : 「緒言」成長・発育期にある小児を対象とする小児歯科領域では, 長期的展望に立脚した的確な診断が必要とされ, その診査項目のなかでX線診査は不可欠なものとなっている. その診査における各種撮影法のうち, 歯・顎全域を1枚の展開像として観察でき, 包括的情報を提供しうる断層方式パノラマX線撮影(以下パノラマX線撮影とする)の小児に対する撮影機会が増加してきている. しかし, この撮影法ももちろん医療被曝をもたらすことは言うまでもない. 医療被曝とは, 放射線の医学利用での患者および被検者の被曝を指し, 人工放射線源による人類の放射線被曝の中で最大である. その上, 自然放射線などとは異なり, 局所的で, 照射野内にある臓器や組織に高い線量を与えるため, 確率的影響としての遺伝的影響や発がんの危険が多大であることから, 特に重要視されている.

Title : Exposure Reduction During Panoramic Radiography in Children 2. Stochastic Risk Estimation from Radiation Dose for Organs

Abstract : Abstract: The present study was carried out for the purpose of exposure reduction during the panoramic radiography in infants, aiming at the field size. The following results were obtained. 1. Making a typical comparison of field size depending on slit difference, a reduction of 28.9% corresponding to 3-year old children was shown. In actual radiography, however, a possible further reduction of 4% was found. 2. In absorbed dose and its equivalents, significant reductions of approximately 55.8% for the lens, 78.9% for the hypophysis, 19.6% for the thyroid, 15.4% for the parotid gland and 13.4% for the submandibular gland-that is an average reduction of 27.7%-were shown, respectively. 3. From the examination of stochastic and deterministic effects using bone marrow dose and absorbed dose, reductions of the induction risks of 33.3% for leukemia and 19.6% for thyroid cancer were found. And otherwise, an average significant reduction of the induction risk of 27.7% for the submandibular gland, parotid gland and other organs was shown. From the above results, it was considered that the use of pediatric slits taking into consideration of age are highly useful to the exposure reduction during panoramic radiography in infants.
