Abstract : 「緒言」歯の動揺度測定は, 歯および歯周組織の健康状態を推測できる有効な臨床的手段である. とくに, 補綴装置を設計する上で支台歯および残存歯の診査診断は, 補綴装置装着後の予後に大きな影響を与える因子の1つである. そのため, 歯の動揺度測定についての研究は古くから多岐にわたり, 外力に対する歯の変位量をダイヤルゲイジ法やストレンゲージ法, また機械的インピーダンス法等により測定してきたが, 臨床応用には数々の問題点があった. その問題点の多くは装置が大掛かりであったり, データの信頼性や再現性が乏しかったりしたもので, 臨床では経験的な感覚を利用したMillerの測定法が日常行われていたのが現状である. しかし, この方法も術者の主観により大きく変化するため, 詳細なデータの記録には適さず, また歯周組織の粘弾性を加味した結果とは言いがたかった.

Abstract : Abstract: For a long time, researchers have been racking their brains to establish an idea method for measuring tooth mobility. However, many measuring instruments so far developed were so intricate that they were of no practical use. In our laboratory, efforts had been made to analyze the degree of tooth mobility from an angle of sound vibration dynamics for this purpose, a clinically working and easy-to-operate measuring system "Dental Mobility Checker" has been completed. Using this model, we have thus far delved into various aspects associated with tooth mobility and published a number of reseach on this subject. We have also carried out dynamic and stress analyses to examine the effects of external force on the teeth, periodontium and prosthetic system. In order to make these analyses useful in clinical practice, we have recently studied the transfer system of external force centered on teeth by taking advantage of out dental mobility measurement technique and stress analysis. The following is a summary of the results of the recent study. 1. The Dental Mobility Checker could work out the degrees of tooth mobility accurately even though changes in the alveolar bone were slight. 2. Dental and maxillofacial stresses could be detected with a non-contact stress analyzer that analyzes the stress distribution in the oral mastication system subject to external force. It was also suggested that the regions on which stresses tend to concentrate are the external anterior nasal aperture and the alveolar process that forms part of the maxillary body. 3. From dental mobility measurements and non-contact stress analysis, it could be surmised that the attached gingivae have something to do with the support of teeth and the stress distribution in the oral mastication system.
