Abstract : 「緒言」根管内に残存する歯髄組織や感染歯質を除去する根管清掃法には, リーマーやファイルなどの切削器具を用いて行う機械的清掃法と薬剤を用いる化学的清掃法がある. 一般的には機械的清掃法に重点が置かれているが, 解剖学的に複雑な形態の根管に対しては化学的清掃法の併用が必要であり, 強力な殺菌作用と有機質溶解作用を有する次亜塩素酸ナトリウムが広く使用されてきた. しかし次亜塩素酸ナトリウムは, 根管から漏洩した場合, 歯周組織や口腔粘膜に対して著しい組織障害を示すことから, 臨床使用に際して慎重さが要求される. そこで, さまざまな分野で消毒薬あるいは洗剤として使用されている界面活性剤の根管清掃剤への応用の可能性が考えられる. 一般的に界面活性剤は, その所要性質として優れた洗浄力, 浸透力, 可溶化性および殺菌効果を有するとされている. 本研究では, 数種の界面活性剤について根管清掃剤への応用の可能性を考察する目的で, 有機質溶解作用, 抗菌作用, および細胞毒性作用として細胞増殖抑制作用, DNA合成阻止作用について検討した.

Title : Foundamental Studies on Utilization of Surfactants as Root Canal Irrigants

Abstract : Abstract: It is difficult to clean completely a root canal which has complicated forms by only mechanical preparation in endodontic therapy, so it is necessary to use root canal irrigants. Sodium hypochlorite has been clinically used commonly as root canal irrigant, but it has toxic action on periodontal tissue or oral mucosa. The purpose of this study was to investigate the effect of some surfactants which were used in many fields as root canal irrigants instead of sodium hypochlorite. The tested surfactants in this study were cationic surfactants (CPC, CAB, benzalkonium chloride, benzethonium chloride), amphoteric surfactants (C14 HSB), and non-ionic surfactants (E.710, E.715). As to the antibacterial effects, the minimum inhibitory concentration (MIC) of surfactants against 20 anaerobic bacteria isolated from human infected root canals and 5 facultative bacteria were examined. The solvent effect on organic debris was examined into dissolving activity using by Type I collagen gel. Further more toxicity on human periodontal ligament fibroblasts (HPLF) was evaluated for cytotoxicity and inhibition of DNA synthesis. The results obtained were as follows: 1. Antibacterial effect of cationic and amphoteric surfactants. And the growth of all bacteria tested were completely inhivited by 0.1% of cationic and amphoteric surfactants as same as 5% sodium hypochlorite. 2. The dissolving action of cationic surfactants such as 0.1% cetyl trimethyl ammoniun bromide and 0.1% benzalkonium chloride were as well as 5% sodium hypochlorite, however effect of amphoteric and non-ionic surfactants were less than half of 5% sodium hypochlorite. 3. Cytotoxicity of 0.1% cationic surfactants such as CPC and CAB to HPLF were less than 5% sodium hypochlorite for the term of 5 minutes. Cytotoxicity of amphoteric and non-ionic surfactants were furthere less effect. 4. Inhibition of DNA synthesis by CAB of cationic surfactants was less than thats of 5% sodium hypochlorite for the period of 5 minutes. And inhibition of DNA synthesis by amphoteric and nonionic surfactants were further low.
