Abstract : 「緒言」歯の喪失と歯槽頂縁との関係において, 何らかの原因で歯を失うと抜歯窩(歯槽)は新生骨によって修復・再生されるが, その後30日~6ヵ月にわたって骨改造現症が起こり, 同時に歯槽頂縁は長期にわたって萎縮性の吸収が起こる. 結果的に抜歯後2~5年以内にその高径を約7mm減じ, その後もゆるやかな吸収が継続するという. その吸収機転が何であるかは現在でも解明つくされていない. また抜歯後に修復される歯槽頂縁の高さは, 何によって決定されるのか, その答は過去の研究者によって示されてはいないが, 我々は抜歯後の治癒の実験から, 抜歯窩口を覆う骨膜(歯槽骨の外面を覆う外骨膜から伸びてきた線維束)の形成されるときの抜歯窩縁の高さが, 最大の要因であろうという結論に達した. つまり抜歯すると, 外骨膜が抜歯窩縁から窩口中央に向かって伸展することによって窩口は口腔粘膜・結合織から完全にシールされ, したがって骨膜より内側(抜歯窩)に骨新生が起こり, 骨膜の外側(上皮側)には骨形成は起こらない.

Title : Positively Bone Regeneration Following Tooth Extraction Using Guided Tissue Regeneration (GTR) Technique

Abstract : Abstract: A sharp decrease in the height of the alveolar crest occuring after the loss of a tooth can be induced by the absorption of alveolar bone. The aim of this study was to develop a technique which can prevent the alveolar crest from the diminution of its height while also positively aiding its growth. The bilateral extraction of the mandibular fourth premolars was carried out in a group of healthy dogs. After removing the gingival flaps, one of the extraction sockets designed as experimental side, was covered with a Gore-Tex membrane forming a tent-like covering on top, while the other side was used as a control by suturing after replacing the gingival flap. The specimens were collected from these animals at 2, 4, 8 and 12 week after the surgery. Histological sections were prepared for both decalcified and non-decalcified tissues, and were observed by light microscopy and by soft X-Ray. The results indicate that an increase of osteogenesis was found along the surface beneath the Gore-Tex membrane as cell proliferation in the lower portion of the gum was protected by the membrane and the covered socket provided better conditions for bone reformation. Increasing the growth of the alveolar crest will greatly facilitate the setting of an artificial denture or in performing a later implantation. Moreover, the deformation of the alveolar crest occuring after extraction can be prevented by covering the extraction socket with the membrane. Thus, we believe that the present method can also be a powerful tool for the clinical use.
