Abstract : 「はじめに」 昭和32年より始まった厚生省歯科疾患実態調査からわが国における乳歯う蝕の動向を観察してみると, 1~15歳児の乳歯う蝕有病率は昭和32年の66.1%から昭和62年の58.8%へと減少している. しかし, 調査年ごとの年齢別df歯数の変化をみると, 減少傾向は昭和56年より鈍化しているのがわかる. さらに, 昭和60~64年の3歳児のう蝕増減傾向を都道府県別にみた結果では, 減少傾向にある都道府県数はわずか7つであり, 逆に12は増加に転じていることも指摘されている. 一方では, 世界保健機関(WHO)と国際歯科連盟(FDI)の西暦2,000年に向けての世界的な歯科保健目標の一つに幼児期をターゲットにしたものがあり, 具体的には"5・6歳児の50%以上がcaries-freeであること"という目標が掲げられている. しかし, 昭和62年の厚生省歯科疾患実態調査の結果によれば, う蝕のない者の割合は5歳児10.09%, 6歳児9.52%であり, 世界的な目標値とは大きな隔たりがある.

Title : Effectiveness of Periodic Dental Health Supervision on Dental Caries Prevention of Young Children

Abstract : Abstract: The purpose of this study was to evaluate a five-time periodic dental supervision (every six months) for young children (succesive supervision from six-month-old to three-year-old) who are at risk of decaying deciduous teeth. This supervision comprised dental health examination, some questionaires at the first visit, a guidance for mother on children's dental health (especially the habits of eating sweets), toothbrushing instruction and professional application of topical fluoride. The samples of the present study were children who were born in 1982 to 1983 and had been living in the same district. We evaluated an effectiveness of the supervision by comparing the results of the final examination at three-year-old. 1,059 children who participated in the supervision were selected as an experimental group and another 1,656 children who didn't participate in the program were served as a control group. The dental caries prevalence rate and dmft index of three-year-old children in the experimental group were 47.6% and 2.64, and those in the control group were 58.7% and 3.54, respectively. The dental caries prevalence rate and dmft index decreased about 19% and 25%, respectively. In the experimental group, dmft index of three-year-old children who received completely a five-time regular dental supervision showed a lower value (2.39) than other children whose number of attendance to the supervision was fewer, and similarly, that of the children who received completely a three-time topical fluoride application showed the lower value (2.14) compared with other children. According to the two questionnaires to the mothers conducted at the first attendance in the supervision and two parameters on oral condition at one-and-a-half year old, there was a significant difference in dmft index at three-year-old. The first question was concerning the nursing habit; breast feeding showed higher value (3.52) than nursing bottle feeding (2.21). The second question was concerning the living with grandfather and/or grandmother ; living together with them showed higher value (4.22) than not living together (2.15). The first parameter was concerning the oral hygiene status; poor habit showed higher value (5.47) than good one (2.21). The second parameter was concerning the classification of caries attack patterns at one-and-a-half year old children; type A showed higher value (8.25) than type 01 (2.43).
