Abstract : 「緒言」 歯科矯正学においては各個人が有する天然歯を素材とし, その位置や軸傾斜をコントロールすることによって咬合の不正を改善するとともに, 正常な下顎の機能を回復し, 各個体毎の正常咬合を完成させることを治療目標としている. したがって, 治療の際には, 個々の歯および歯列弓の形態や咬合状態を正確に把握する必要がある. 今まで, 咬合再構成を行うに際し, その歯のもつ機能を十分発揮しうるか否かを決定する意味で, 歯軸が非常に重要な一要素であると考えられている. そのため, 歯軸に関しては数多くの研究報告がなされている. しかし, 下顎運動を考慮した咬合再構成を行う際には, 歯軸だけでなく, 上顎の歯の位置や軸傾斜によって変化する歯冠表面に存在する歯の誘導路も重要な一つの要因であると考えられる. そして, 歯の誘導路は立体的な下顎運動と関係しているため, 顎口腔系, いいかえれば, 歯, 歯周組織, 顎関節, 神経筋機構ならびに脈管系の相互作用をも考慮して適切に位置づけなければならない.

Title : A Study of Occlusal Form in the Class II Div. 1 Malocclusion Subjects

Abstract : In order to orthodontically reconstruct the occlusion, it is very important to understand the interrelationship among the functional condylar movement, the occlusal guidance and the morphological features of dentofacial skeleton. However, it has not yet been established any specific index for providing the proper occlusal guidance in orthodontic diagnosis. In this study, the inclination of occlusal guidance in maxillary dentition in the normal occlusion and malocclusion subjects was examined. Subjects consisted of fifty-eight normal occlusion and twenty-seven Class II div.1 malocclusion with no severe imbrication of the dentition. The maxillary casts which were mounted and related to axis-orbital plane (AOP) on SAM2 articulater and records of condylar path during protrusive movement from computerized axiograph (cadiax : Gamma Co Ltd, Austria) were available to assess the inclinations of the occlusal guidance and wear facet, and the horizontal condylar inclination (HCI). The measurements for the inclination of occlusal guidances and wear facets were achieved by means of a 3D-digitizer (Gamma Co.Ltd, Austria). Dentofacial structure of the subjects was also examined utilizing the tracings of rentogenocephalometric X-Ray films. Inclination of the occlusal guidance was decreasing from anterior to posterior teeth in both normal occlusion and Class II div.1 malocclusion subjects. Class II div.1 malocclusion have a low possibility to disclude during protrusion and laterotrusion movements due to the results that the inclinations of occlusal guidance in Class II div.1 malocclusion was flatter in the anterior segment (incisor) and was steeper in buccal segments (premolar) than that in normal occlusion. Thus, it was suggested that Class II div.1 subjects have a tendency to act as a group functioned guidance. Relatively high appearance of wear facets in Class II div.1 subjects was seen in incisor, canine, and first premolar. Low appearance of wear facets in lingual cusp in Class II div.1 subjects was also observed indicating that the steep guidance of laterotrusion provided the much of disclusion in mediotrusion side. There were correlation among the dentofacial structures of Class II div.1 subjects and the guiding plane inclinations upper central incisor and canine. These findings indicated that a form of occlusal guigance is closely related to the dentofacial skeleton and individual guiding planes. Assessment of guiding planes and wear facets in each subjects provide important informations for proper diagnosis and treatment planning of malocclusion.
