Abstract : 「緒言」 歯周疾患発症の局所的有害因子には微生物的因子, 化学的因子, 温度的因子, 放射線的因子ならびに機械的因子があげられる. とくに補綴処置と関連が深い機械的因子については食片圧入や咬合性外傷があげられ, Hirschfeldは咬耗, 隣接サポートの消失, 歯の廷出, 先天性形態異常ならびに不適切な歯冠修復物は食片圧入の原因と報告している. これらの中でも歯の隣接接触関係は咬合や歯周組織の保全に大きな影響を及ぼす因子と考えられており, 歯の接触点の喪失は歯槽骨の吸収や歯列の乱れを生じる大きな原因の一つと言われてきた. 歯の隣接接触関係については, その位置, 形態および大きさなどの形態的解明や機能的問題の一つの解明法として接触点強さの研究が行われ, その結果の一部が臨床に応用されている. また, 更に詳細な機能的分析や隣接接触機能の分析として, 接触関係における歯の3次元的動きや接触点の衝撃減衰能力の解析などが報告されているが, 一致した見解を持つまでには至らない.

Title : The Effects of Loss of Contact Relationship on Stress Distributin in the Mandible

Abstract : Abstract : A few studies were reported on the relationship of tooth contact to supporting tissues analyzed in terms of induced stress. In the present study, the authors analyzed the changes in stress produced in teeth and the mandible when the tooth-to-tooth contact relationship was lost. The instrument used in this study was a non-contact dynamic stress analyzer which bases its principle on thermal elasticity effect. The material was a human dry mandible with a 0.3mm thick layer of artificial periodontal membrane. An exciting force was applied to the right-side first molar (6) alone or the molars on the right-side of the lower arch all at once. Before stress was measured, the points of contact were cut first in the first molar mesially and distally, and eventually the all the contact points of the proximal surfaces of adjacent molars were eliminated. The results were as follows. 1. The loss of contact point resulted in increased and expanded amounts of stress in the tooth, the alveole, the mandibular body and the mandibular condyle. 2. when an exciting force was applied to a set of teeth simultaneously, it was suggested that the effects of contact point loss are likely to appear than when the force was exerted on a single tooth.
