Abstract : 「緒言」 現在, 歯科医学の分野では, いわゆる顎関節症に対する認識の高まりから, 顎運動と調和した機能的な咬合を再構築することが重要な課題となってきている. とくに顎関節の機能障害や基質的な変化を伴った症例では, その病態を的確に把握しているかどうかが咬合治療の成否に大きな影響を及ぼすと考えられる. このような顎関節症では, 顎関節雑音が高い出現率を示すが, この雑音には顎関節の病態を反映している情報が含まれていると考えられていることから, その診査の重要性が指摘されている. 日常臨床における顎関節雑音の簡便な診査方法には, 触診および聴診があげられる. しかし, これらの方法は, 術者の主観に頼るため, 術者間で評価が異なることが多いという欠点を含んでいる. そこで, ME(medical electronics)機器を用いて顎関節雑音を定量的に評価しようとする研究や, 顎関節雑音の客観的な評価から顎関節の病態を推測しようとする試みがなされているが, それらは, 未だ不確定要素が多く確立された方法とは言い難い.

Title : Relationship of Temporomandibular Joint Sounds and Condylar Movement to Disease of the Temporomandibular Joint

Abstract : Abstract : The present study was undertaken to clarify the relationships of objective features of the temporomandibular joint (sounds and tracks of the condylar movement) to diseases of this joint. The subjects were 25 patients whose temporomandibular joints (26 in total) were found to produce sounds. Using an electric capacitor microphone and a computerized axiograph, temporomandibular joint sounds during opening and closing, and the tracks of coundylar movement were recorded simultaneously. The relationship of these records to abnormalities of the temporomandibular joint, as assessed using MRI, was analyzed. The assessment of temporomandibular joint abnormalities using MRI includes the presence or absence and severity of articular disk displacement, the presence or absence of lateral displacement of the disk, and the presence or absence of morphological abnormalities of the disk, condylar head and articular eminence. The maximum amplitude and site of origin of the sounds varied depending on the presence or absence of the disk displacement. The sound-producing area, and its percentage of the total area on which the condylar slides, varied depending on the severity of the disk displacement. The cumulative frequency of sounds and the magnitude of condylar head sliding in the sound-producing area tended to vary depending on the presence or absence of lateral displacement of the disk. When morphological abnormalities of the disk were analyzed in detail, the duration of sounds differed among disks with different morphological features. The cumulative frequency of sounds also varied among condylar heads with different morphological abnormalities. Furthermore, the maximum amplitude of sounds differed depending on the presence or absence of morphological abnormalities of the articular eminence. These results suggest that the features of temporomandibular joint sounds and the tracks of condylar movement are related to the presence/absence and severity of the disk displacement, the presence/absence of lateral displacement of the disk and to organic abnormalities of this joint. It is also suggested that simultaneous measurement and analysis of temporomandibular joint sounds and tracks of condylar movement permit estimation of abnormalities of the temporomandibular joint to some extent.
