Abstract : 「緒言」 局部床義歯による歯科医療の最終目的は, 顎口腔系に機能的で審美的な咬合関係をいかにして与える事である. そのための条件としては, 支台歯の負担能力からみて可及的小さい垂直的応力で高い咀嚼能率を上げること, ならびに側方圧をできるだけ小さく抑えられる咬合関係の実現に勤めなければならない. その他に咬合以外のさまざまな要素があるが欠損補綴にあたっては, いかなる咬合を与えるかが大きな問題となる. 特に混合負担様式をとる遊離端義歯にとっては重要な問題であろう. 義歯の咬合時の構造力学的解析は, Gysiが人工歯に加わる力の解析を行ったのをその初めとして多種多様な研究がなされ, また近年では有限要素等を用いた宮川, Aydilik, 川崎らの研究報告や当教室一連の実験である義歯や支台歯に加わる力の解析や安定性に関する実験報告が数多くなされている. しかし, いずれも中心咬合位のみを想定したものであり, 側方運動時を想定した実験の報告は殆ど無い.

Title : The Influence of Different Occlusal Types to Denture Movement and Abutment Teeth Displacement

Abstract : Abstract : The removable partial denture is to render functional occlusal relationships to the stomatognathic system. Especially in placing free-end saddle dentures, which are liable to tilt or be displaced under pressure, the choosing of occusal types is of cardinal importance. Many researchers have come up with analytical findings about forces exerted on dentures and abutments and about it stability thereof. However, their attention has primarily been focused on centric occlusion. There are scarcely any report of experiments with side shift taken into consideration. Such being the circumstances, we thought it necessary to go into the effects of occlusal types on denture stability and the amount of stress on abutment teeth during lateral movements of the mandibule. In the present study, we applied the three proposed types of occlusion - bilateral balanced occlusion, unilateral balanced occlusion and mutually protected occlusion - in lower bilateral free-end saddle dentures, and examined the activities and displacement of the denture and abutments under dynamic load using a modal analysis. 1. In all the samples used in this experiment, two vibration modes were observed, and there was no difference between the three types of occlusion. 2. Upon modal analysis, it was found that during lateral movements, the forces directed against abutment teeth were reduced in either bilateral or unilateral balanced occlusion. With the mutually protected occlusion, on the other hand, the abutment teeth bore the largest load probably because the lateral forces were exerted on the canine and first premolar alone. In the centric occlusion, the deformation and displacement of dentures were the smallest. 3. Upon the displacements of abutment teeth, the largest lateral pressure (5.75mm/N) was applied to the abutments with the mutually protected occlusion. The lateral pressure of bilateral balanced occlusion was 2.54mm/N, and that of unilateral balanced occlusion was 2.63mm/N.
