| Abstract ： | 「緒言」 矯正治療を希望して神奈川歯科大学附属病院矯正科を訪れる患者のうち, 反対咬合を呈するものはその約40%を占めている. 一方, 日本人の反対咬合は, 上顎骨の劣成長に起因するものが多いことが明らかにされている. これらの骨格性反対咬合に対する治療法のひとつとして, 近年, 上顎に積極的に顎整形力を加えようとする, いわゆる上顎前方牽引が行なわれるようになった. これには, 上顎を前方牽引するだけのものと, 下顎を後方へ牽引するオトガイ帽装置に付加して使用するものとがあり, 前者はマスクタイプ, 後者はホーンタイプあるいはボウタイプと呼ばれている. しかしいずれのタイプも, 患者の顔の前方部に付加装置があるという特殊な形態およびその外観から, この装置の1日の着用時間はごく限られてしまい, 患者および保護者の治療に対する理解と協力が得られなければ, 折角の効果が半減してしまうのが現状である. |

| Abstract ： | Abstract : This study clinically carried out to evaluate the effects of the modified sliding yoke with chin cap compared with the maxillary protracting appliance on the skeletal Calss III malocclusion. Ten out of twenty skeletal Class III cases with under growth of the maxillary complex during growth period (Hellmann's dental age III B-III C) used one appliance and the rest used the other. And then cephalograms of these cases before and after the treatment were analyzed. The findings were as follows : 1) It took as long to improve over jet using the modified sliding yoke with chin cap as it did when using the maxillary protracting appliance. 2) In using the modified sliding yoke with chin cap counter-clockwise rotation of the maxillary complex, elongation and mesially tipping of upper molars were not seen so much as the maxillary protracting appliance. 3) Other effects of the modified sliding yoke with chin cap were almost the same with those of the maxillary protracting appliance. These findings suggest that it is effective to use the modified sliding yoke with chin cap for the skeletal Class III malocclusion with under-growth of the maxillary complex during growth period. Therefore, it is possible to say that the modified sliding yoke with chin cap can be used for the patients who can not use the maxillary protracting appliance. |
