Abstract : 「はじめに」 現在, 厚生省ならびに日本歯科医師会は8020運動を推進中であるが, これを実現する上で, 歯の喪失の主要な原因となっているう蝕と歯周疾患の予防に有効なブラッシングを奨励することも重要な戦術の一つである. しかし, ブラッシングにはオーバーブラッシングという問題も含まれている. この要因として, ブラッシング圧, ストローク幅, ブラッシングの回数および時間, 歯みがき方法などが考えられている. そして, このオーバーブラッシングは, 歯肉の退縮やくさび状欠損などの原因の一つといわれている. ことに, 歯頚部のくさび状欠損とオーバーブラッシングの関係は以前より議論されてきており, ブラッシングそのものを主要な原因とする意見が多い. しかし, この他にも, 歯ブラシ当てすぎ説, 酸蝕やそれらの複合説, エナメル小柱のはじき説などがある. しかし一方では, 歯ブラシを使用したことのない人にも磨耗による実質欠損が見られるという報告もあり, 必ずしもブラッシングが最大の要因となっていることもなさそうである.

Title : Prevalence of Wedge-Shaped Defects in Recent Years

Abstract : Abstract : Wedge-shaped defect occured in the cervical area is one of the major dental problems. Several surveys on the prevalence of this defect were carried out about fourty years ago. It has been generally believed that the levels of the defect in females on left side of jaw were higher than in males on right side one. We can't get, however, the recent informations of the defect. The present study has been carried out from 1994 for the purpose to assess how the recent prevalence of wedge-shaped defect in Japanese adults. At a regular dental examination the survey including the detection of the defect was conducted on 1,255 persons aged 18-66 (average 34.9 years, 993 males and 262 females) working as employees at a certain enterprise. The results were summarized as follows ; 1) The wedge-shaped defect was observed in 27% of the subject of the survey ; 29.2% in males and 18.5% in females. 2) It was shown that the prevalence of the defect was increasing with age and was higher in the upper teeth than in the lower teeth. 3) There wasn't any informations about the difference of this prevalence between right and left side of jaws. 4) The prevalence of the defect in various tooth groups showed a constant characteristics to the previous results ; the prevalence in premolars and cuspid teeth was considerably higher than that in other tooth groups, however, that in second molar was very low. All things considered it was suggested that the major cause of the defect may be connected with toothbrushing force.
