| Abstract ： | 「緒言」 局部床義歯の欠損様式中で, 一番問題点が多いのは遊離端欠損と言われている. これは臼歯が欠損することによりcentric stopが減り, 咬合支持(occlusal support)が少なくなって咬合が不安定になるためである. 特に片側の遊離端欠損は左右の顎機能の調和が崩れ, 慢性化すると顎関節症をひきおこしやすくなる. Andersonらの調査によると遊離端義歯は義歯装着者の中で使用されない率が最も高いと報告している. これらは咀嚼機能の不良, 支台装置による審美的不満, 連結(部)装置からくる発音障害などから未装着者が多いと推察できる. これら遊離端義歯の問題点を解決するために多くのテクニックや支台装置(アタッチメント)が開発され, 設計されてきた. 支台歯と欠損部顎堤に咬合力が効果的に配分されるには支台歯と義歯が強固に結合していなくてはならず, 支台装置としてprecision attachmentやtelescope crownが有効とされてきている. |

| Abstract ： | Abstract : The most difficult process making a denture is free-end missing case. Because of the posterior teeth were missing and the centric stops were reduced, then the occulusion became instabilized. The unilateral free-end missing cases breakes the harmony of right and left temporomadibular joints function, and if it had been chroniced it causes arthosis of the temporomandibula joint. In this investigation, three free end missing were chosen. The attachments were inserted as the retainers. And the changes of edentulous areas, the mobility of abutment teeth, the generation of new caries, and concurrence of periodontal disease were observed while several years after insertion of the attachments. The observation from stable stage to senile changes stage of human being was indicated followings. 1. The phenomenon (e.g. resorption of albeoral bone, generation of new caries, etc.) progressed faster from senile change stage. 2. Abrasion and fatigue were observed on the construction materials of denture (except metal) from 5 years. 3. The CEKA attachment had almost no trouble about the difficulty of insertion or taking off and breaking down. |
